I received “ONE” e-mail from you on 2-1-09 (a month after the incident took place) and forwarded that e-mail to
the appropriate dept. Your claim has been turned over to our insurance company for review and they will contact
you. If you have any questions you have our number and you are welcome to call, or you may take any other
steps you feel are necessary.

Keith Marvin,
I just wanted to let you know Lance Smith has not responded to either of the two emails I have
sent, regarding the incident that took place December 30, 2008. Before I escalate this to the next
step, is there any other person you may know or have contacts with and I may contact at Tiseo? If
I don't hear from someone at Tiseo Paving Inc. by Wednesday Feb 11, 2009, I will take it to the
"next step".
