Brad,
I would have been the person to approve the permit, which I remember doing. However, I just checked with the Admin
assistant and we do not have a record of the permit in the filing system.
Does anyone on site have a copy of the permit? The only thing I can think of is that permit was picked up without it being
copied in our records. Attached is the visual that accompanied the cover sheet, so we did have it in our office.
If they provide another cover sheet, I can quickly approve it, again.
