Qualifications: Subgrade must be +/-.01 per contract. Densities for subgrade must be provided. Five foot each side
of paving is required for access. Per contract slip forming only that portion under contract
see station numbers above. Layout points required.
Mix Designs for slip forming and hand pour are attached for approval. Concrete orders place on each
Monday at 10:00 am for the next week, Written approval and acceptenco is required prior to
releasing work to proceed. Seven working day notice required for scheduling for machine.
Scheduling does not include rain days. No retainage on slip form operation.
From Sta. 4 +19 to Ste. 1+00 slip form width will be 25', Hand pour curb /gutter and balance of
street starting from 18" at Sta. 4 +19 to 4' at Sta. 1 +00 on each side of the road.
Please issue a Change Order at your earliest convenience. Sign and return copy showing your acceptance.
