A walk-thru on the above referenced project was conducted on
12-16-96 and the items below were found to be deficient.
Fire Hydrant #1 Start at Denton Tap.

Fire Hydrant #1 has a mainline valve next to it. Needs valve stack replaced
(stack and cap.)
F.H. #2 needs a new pad poured.
F.H #3 leaks at bottom section.
Main valve #2 on corner of 553 and Ridgemont. Can't get on valve.
Needs to be raised to ground level.
F.H. #4 leaks at Bonnett. Repair
F.H. #6 needs new pumper cap. Leaks at seal.
F.H. #10 Leaking from bottom (below ground).
F.H. #11 corner of MacArthur and East bound 553
Leaking at bottom (below ground)
F.H at Lake Vista and MacArthur leaking below ground,
At Peninsula and bridge on MacArthur- locate blow-off(.2"),
From Denton Tap to MacArthur line has only two Main Line Valves.
LINE A
MH a, x 32 needs chimney seal installed properly (to bottom of Ring) also needs
an extension for the rest of the chimney.
MH At 3:2 bottom of seal still loose tots of flow on line for nothing to be on this
iine.
MH 10 + 01 top of seal not installed properiy to ring and bottom loose also
needs dirt North side of MH.
LINE D
MH 0 + 00 needs new ring cover, rein pan, chimney seal, and leak in MH
repaired.
MH N. of 0 + O0 needs chimney seal properly installed ( to bottom of ring) and
extension for rest of chimney (Between 0 + 00 and 5 + 00)
MH 5 + 00 compaction bad, leaking in MH N. side at bottom 4 inches above
floor,
MH 10 + O0 chimney seal needs to be installed properly ( to bottom of ring)
looks like it has been leaking 4 inches from bottom S. side.
MH 10 + 66 needs chimney seal tightened on bottom,
LINE E
MH 4 + 15 is too high (higher than curb) Chimney seal loose on bottom.
Leaking above outlet pipe to East at Ridgemont S. side.
LINE F
MH 2 + 55 Chimney seal loose on bottom.
MH 5 + 77 W. side Denton Tap, Bottom of chimney seal loose. Stub out to
North is White PVC. Replace it with green SDR35.
VISTA RIDGE AREA
1st New-MH east on Denton Tap New-Chimney seal does not meet
the ring and bottom loose, looks like seam is leaking.
PIP 2nd Exist-MH needs new ring and not Lewisvilles lid. Rain pan chimney
seal and levee (S3 Stake)
PIP 3rd Exist-MH needs new ring and cover, chimney seal, Rain pan.
PIP 4th Exist-MH needs new ring and cover, chimney seal, Rain pan.
PIP 5th Exist-MH needs new ring and cover, chimney seal, Rain pan. (before
drainage where bridge is on 553) leaking at drop about <5gpm.
PIP 6th Exist-MH needs new ring and cover, chimney seal, Rain pan.

PIP 7th Exist-MH needs new ring and lid, chimney seal, Rain pan.
PIP 8th Exist-MH needs new ring and cover, chimney seal, Rain pan.
