Ms. Fassett, I am preparing paperwork for payment of this order and need additional information to add your firm to our
financial system. It is my understanding that there are approximately 14 plaques to be created for the Rolling Oaks
Cemetery grounds and that each one costs $1,944.00. Please correct me if this is an error.
I will need your federal tax ID number and the official name you want us to use for payments (i.e. Matthews Intl. or Pittsburgh
Architectural), the mailing address for remittances and an estimate of shipping costs for each item (if you can) since
shipping is not included in the price of the plaques.
I want to get your purchase order set up as soon as possible, so the sooner I get your info, the sooner the first payment can
