Dear Ms. Cantu:
This letter is to inform you that work performed under the above referenced work order number
is incomplete according to City of Coppell standards. The deficiencies remaining to be corrected
are as follows:
1.Additional dressing up in disturbed areas to restore to previous condition.
2.Uncovering buried water valves (in at least two locations) with the help of Danny Hightower,
Public Utilities Dept.
3.Walk-through inspection for Completion Verification Notification (page 4 of permit form).
Engineering Department representatives have spoken to the contractor and SBC personnel on
numerous occasions regarding City of Coppell requirements with no apparent response. Please
call me at your earliest opportunity to discuss the steps necessary to close out this permit.
