SBC-CS041208 December 8, 2004 Lisa Cantu SBC Communications 1460 Roundtable Dallas TX 75247 RE: ROW Use Permit for Freeport Parkway and Creekview Drive (JP Morgan) Dear Ms. Cantu: The City of Coppell has received and reviewed the plans submitted for an installation at the above referenced location and we have the following comments to offer: 1.The permit application does not include contractor information. The City will require the contractor name, contact person, and 24-hr. emergency contact number prior to issuance of the permit. 2.Contractor shall pay special attention to pages 3 & 4 of the permit application. The City of Coppell will require conformance to all items listed on page 3. Page 4 is to be completed by City Streets and Utilities divisions personnel and returned to this office prior to release of any additional permits to SBC or the contractor involved. 3.Maintain a minimum 5’ horizontal clearance from City water, sewer or storm drain facilities. 4.Maintain a minimum 2’ vertical clearance, preferably under our waterlines as opposed to above. There are areas where you show 60” of cover on a waterline and your installation is 42” deep. In those instances we require you to go under the waterline. 5.On Creekview Drive (print 11) where you cross in front of the existing retaining wall – any damage to that wall caused by this work will be the responsibility of the contractor. I strongly suggest you take photographs of the wall prior to working in this area. 6.Several locations along Creekview Dr. where existing utilities say “verify cover” we expect a minimum 2’ separation under that existing utility. This permit can be approved with these referenced requirements as conditions, however the permit cannot be released until contractor information is provided.
