Enclosed for your review and action are items to be addressed prior to acceptance of the
referenced project.
1. Sta. 42+52.27 grout needed, rain pan, chimney seal. Abandon existing manhole.
2. Sta. 40+35.00 rain pan, chimney seal, line is very dirty.
3. Sta. 35 +40.41 rain pan, chimney seal, inflow on incoming invert.
4. Sta. 33+47.78 grout manhole, chimney seal, holding 1/3 pipe of water, all seams need
grout for inflow.
5. Plans show that manhole coming from Burns Road should still be in place.
6. Sta. 30+46.68 raise to grade, grout, chimney seal, rain pan.
7. Sta. 25+47.35 grout manhole, chimney seal, remove sand bag. What is line coming in
from S.W. 2 feet off floor, line needs cleaning.
8. Sta. 20+56.00 chimney seal, poor compaction, seam leaking where 4 feet meets 5 feet.
Need class 400 lid and ring installed.
9. Sta. 16+36.00 grout, chimney seal, re-center ring and lid.
10. Sta. 12+10.00 grout, chimney seal, re-center lid and ring, clean pipe and floor, crack in
wall.
11. Sta. 8+54.53 (.57) chimney seal.
12. Sta. 5+41.02 chimney seal 26 in of riser. Rings make manhole neck too long.
13. Manhole before inverted siphon saw no Sta. # chimney seal torn and has no lower band,
invert needs work badly, plug old line.
14. Sta. 1 +62.33 missing bolts, flow needs to be diverted into siphon box. Get old manhole
lid and ring out of creek bottom.
15. Sta. 0+40 needs gaskets on bolts for bolt down.
16. Sta. 0-1-08 needs ring reset to cone.
17. Abandon manhole at Sta. 24.50 4-.
18. Plug manhole at Mitchell Road.
19. Adjust manhole at Sta. 204.56 and 124-10.
20. Failures at Sta. 204-56 and 144-00.
21. Replace traffic signal conduit at Coppell Road to the west.
22. Repair and clean out culvert at 417 Bethel.
23. Replace mailbox at 412 Bethel Road.
24. Haul off debris and rubble.
25. Switch over to siphon.
26. Produce "as built" plans.
Please advise as to when this work will be done so that we may have a representative present.
