Please refer to the attached pages for information related to the completion of franchise
permits in the City of Coppell. Your company is expected to adhere to the requirements
listed on Page 3 of the ROW Use Permit Application form. Please pay particular attention to
Numbers 15-17 on the checklist. You are also required to complete Page 4 of the ROW Use
Permit application form (attached) by having the work area inspected by City of Coppell Utility
Dept. personnel.
The work activity performed under the referenced permit number(s) issued June 14, 2004
has left a hole that needs to be back-filled and construction debris that must be cleaned up in
the vicinity of Riverchase Drive near MacArthur Blvd. If you need details about the location
where the work needs to be done, call me at 469-576-7709.
Please notify the Engineering Dept. by faxing the Completion Verification Notice after the
clean-up is complete and the inspection has been done. Thank you for your prompt attention
to this matter.
