Attached is a report that includes an application for a FEMA Conditional
Letter of Map Revision based on the placement of fill on a 2.36 -acre property
composed of tracts I -V. This property is located within a reach of Denton
Creek That extends north of Sandy Lake Road and east of Starleaf Street. The
mapping revision ties to current FEMA FIRM 48113CO155J just outside the
property boundaries.
We have also included a CDC permit application since this property is
traversed by Elm Fork river stationing 93359 and 93940. The loss of valley
storage is to be replaced with a proposed swell that includes portions of
tracts II -V.
Please review these submittals and if acceptable sign sections A and B of the
CLOMR -F application on the second page of the MT -2 Form 1, and complete the
Final CDC Action /Findings Form. We can pick up a signed copy of the CLOMR -F
application and forward it on to FEMA with the required fee for processing.
