Hereby requests and authorizes Craig Olden, Inc. to perform the following services:
SCOPE OF SERVICES:
1. FIELD INSPECTION — Perform up to three (3) inspections of gabion and rock riprap installation. One
inspection will be performed upon Contractor's completion of 10' x 10' rock riprap test area (in accordance
with plan note 6.4). Another inspection will be conducted during early stages of installation of gabions.
Depending on construction sequencing, a third inspection will be made during installation of the production
rock riprap.
2. INSPECTION REPORT — A brief written inspection report will be generated and submitted to the City
following each inspection.
Compensation to be on a Lump Sum basis equal to One Thousand Five Hundred and No /100
Dollars ($1,500.00) per inspection event, for a total allowance of $4,500.00 for three (3)
inspections.
PROVISIONS
Services covered by this authorization shall be performed in accordance with the following PROVISIONS.
1. Payment for services will be due within 30 days of invoice.
