Not too sure what could have happened to the rest of it. I double checked

contained 2 copies of the signed final pay request, consent of surety to
final payment and contractors affidavit to final payment.

We recieved the signed change order for the additional concrete on this
project. We still need the final pay request and affidavit of final payment
so we can close out the project. Please forward these items to me so I can
get them processed.
