Pertinent information about the request is listed below. If we do not receive the required data within 90 days, we will suspend our processing of your request. Any data submitted after 90 days will be treated as an original submittal and will be subject to all submittal/payment procedures, including the flat review and processing fee for requests of this type established by the current fee schedule. A copy of the notice summarizing the current fee schedule, which was published in the Federal Register, is enclosed for your information. FEMA receives a very large volume of requests and cannot maintain inactive requests for an indefinite period of time. Dixon, P.E., CFM Communities: Cities of Carrollton, Coppell, and Community Nos.: 480167, 480170, and 480171 Dallas, TX The issues listed below must be addressed before we can continue the review of your request. 1. Our review revealed that, along with the City of Coppell, the Cities of Carrollton and Dallas are also affected by this revision. Please provide a letter of community acknowledgment stating that the Cities of Carrollton and Dallas have reviewed the revision request, understand the effects of the revision on flooding conditions in their communities, and determined that any land and any existing or proposed structures to be removed from the Special Flood Hazard Area, the area that would be inundated by the base (1- percent - annual- chance) flood, are or will be reasonably safe from flooding, or provide a completed Application/Certification Form 1, entitled "Overview and Concurrence Form," signed by officials from the Cities of Carrollton and Dallas. Alternatively, if the revision area has been annexed by the City of Coppell, please provide copies of the annexation agreement and a current corporate limits map. 2. Our review revealed that a floodway analysis was not performed for the duplicate effective, existing and proposed conditions models. Please revise the duplicate effective, existing and proposed conditions HEC- RAS models to include an analysis of the regulatory floodway. Please provide paper and digital copies of the input and output files for these models. Please also ensure that the surcharges do not exceed the 1- foot maximum allowed. 3. The submitted topographic work map entitled, "Proposed Conditions Floodplain Workmap," prepared by Cotter Associates, LLC, and dated August 2010, does not provide essential information required to enable us to complete our review of this request. Please submit a revised topographic work map, certified by a registered Professional Engineer, that shows all applicable items listed in Section C of Application/Certification Form 2, entitled "Riverine Hydrology and Hydraulics Form," including: a. Continuous boundary delineations of the proposed conditions base (1- percent - annual- chance) floodplain, 0.2- percent - annual- chance floodplain, and regulatory floodway b. Continuous boundary delineations of the currently effective base floodplain, 0.2- percent - annual- chance floodplain, and regulatory floodway c. Logical tie -ins between the revised and effective flood hazard boundary delineations d. Topographic contour information used for the boundary delineations of the base floodplain and 0.2- percent - annual- chance floodplain, with legible contour labels for the entire revision area e. Please submit the annotated Flood Insurance Rate Maps (FIRMs), at the scale of the effective FIRMs 48113C0155 J and 48113C0160 J, that show the revised boundary delineations of the base floodplain, 0.2- percent - annual- chance floodplain, and regulatory floodway shown on the submitted work map and how they tie into the boundary delineations shown on the effective FIRM at the downstream and upstream ends of the revised reach. Effective January 13, 2010, the Federal Emergency Management Agency revised the fee schedule for reviewing and processing requests for conditional and final modifications to published flood information and maps. A copy of the notice summarizing the current fee schedule, which was published in the Federal Register, is enclosed for your information. In accordance with this schedule, the fee for your request is $4,400, which must be submitted before we can continue processing your request. Payment of this fee must be made in the form of a check, money order, or credit card payment (Visa or MasterCard only), payable in U.S. funds to the National Flood Insurance Program. For identification purposes, the case number referenced above must be included on the check or money order. We will not perform a detailed technical review of your request until we receive this payment.
