LMC has no knowledge or authority regarding where this mixture is to be placed, therefore it is the direct responsibility of the Purchaser, the design
architect or design engineer, and or contractor to insure that the above designed mixture parameters for compressive strength, mixture proportions or
ratios, and other required performance parameters are appropriate for the anticipated environmental, durability, or usage conditions in ACI 318 Chapter
4 and/or the relevant local building codes.
LMC warrants the supplied ready -mix concrete mixture designs which LMC guarantees will achieve the specified compressive strength, or other
performance properties of the concrete stated in the Contract Documents, as were made available at the time of quotation, when tested and evaluated
in accordance with ASTM C172 -99, ASTM C311C31M -03a (including proper initial curing as per Section 10.12), ASTM C391C39M -01, ASTM
C941C94M -07, or other standard test methods indicated in the Contract Documents that were necessary to verity the additional performance properties
of the concrete, and when the recommended procedures for placement and curing of the in -place concrete, or those specifically outlined in ACI
305R -99 and ACI 3068 -88 are followed, if and when necessary. Additional information can be found in the Limited Warranty for Ready Mix Concrete.
In accordance with ASTM C941094M -07 Section 4.6, Lattimore Materials Company shall receive all copies of testing reports, and they should be
forwarded to our Quality Control Department (fax 972.221. 9647) as soon as available. Failure to forward reports in a timely fashion can delay response
time in dealing with concrete concerns as well as future project submittals.
This mix design submittal, price quote, or material specific information (aggregate properties, grading, material composition, or other information
regarding materials manufactured by Lattimore Materials Company) is proprietary and confidential and not to be shared or transmitted in any form to
any person or organization that is not expressly authorized in writing by a designated official of Lattimore Materials Company. Any unauthorized person
or entity in possession of this information will be prosecuted to the fullest extent of the law.

THIS LIMITED WARRANTY SUPERCEDES ALL PRIOR W ARRAN TY COVERAGE
Lattimore Materials Company, L.P. (TAIC ") hereby extends to the Purchaser of its supplied ready -mix concrete a ]united
warranty for the reads -mix concrete mixture design a specific performance deficiencies subject to the terms and conditions
set forth below. LAIC shall warrant that, for a period of one (1) rears, beginning on the date of concrete placement at the
original delivery location ("Warranty Period "), LXIC will remedy, repair, or replace non - conforming concrete as prodded in this
Limited Warranty. The liability of I-XIC under this Limited Warranty shall be limited to the remedy, repair, or replacement of
said non - conforming concrete as provided in this Limited Warranty and LhIC "s total liability- shall not exceed the direct expense
associated with remedy, repair, or replacement of said non - conforming concrete.
LAIC has no knowledge or authority regarding where this concrete mixture(s) is /are to be placed, therefore it is the direct
responsibility of the Purchaser, the project architect or engineer, and or contractor to insure that the above designed mixture
parameters for compressive strength, mixtur proportions or ratios, and other required performance parameters are appropriate
for the anticipated environmental, durability, or usage conditions in ACI 318 Chapter 4 and /or the relevant local building codes.
PLEASE READ CAREFULLY THE SCOPE AND TERMS OF YOUR LIMITED WARRANTY
WARRANTY
During the term of this Limited Warranty, and subject to the terms and conditions listed herein, LA-IC warrants the
supplied ready -mix concrete mixture designs which LAIC guarantees will achieve the specified compressive strength, or
other performance properties of the concrete stated in the Contract Documents, as were made available at the time of
quotation, when tested and evaluated in accordance with AST11I C172 -99, ASTAI C31/C312\I -03a (including proper
initial curing as per Section 10.1.2), ASnl C39/C39M -01, ASnl C94/C94111 -07, or other standard test methods
indicated in the Contract Documents that were necessary to verify the additional performance properties of the
concrete, and when the recommended procedures for placement and curing of the in -place concrete, or those
specifically outlined in ACI 305R -99 and ACI 306R -88 are followed, if and when necessary.
This -ill also serve as a Limited Warranty that the compressive strength, or other performance properties of the
concrete as specified in the original Contract Documents for the concrete supplied to the project, will meet or exceed
those requirements during the one (1) year Warranty Period from when the concrete vas originally supplied to the
Purchaser. The Limited Warranty applies only to the original Purchaser of the supplied ready -mix concrete.
LL%IITATIONS
This Limited Warranty and guarantee does not and will not cover any claims or liabilities related to the following items,
which are expressly excluded from the coverage provided by this Limited Warranty-:
1. Drying shrinkage cracking, plastic shrinkage cracking, or other cracking or cracking process. LNIC guarantees
that concrete -ill crack as a normal function of the materials, due to normal processes of expansion and
contraction, and because of environmental conditions that are outside of the control of LA-IC.
2. Concrete surface defects or imperfections.
3. Aesthetic blemishes or discoloration in the concrete.
4. Deterioration to the concrete as a result of improper, incorrect, or poor finishing practices.
5. Defects caused by normal -wear and tear.
6. Materials contained vTithin the concrete that are within allowable limits according to AST1I C33 -03, ASTM
C94/C94NI -07, AST i11 0150 -07, ASn-I C260 -01, ASTAI C494/C494NI -01, AST i41 0618 -03, ASn-I C1157 -03,
and ASTAM C1602/CI602NI -06.
7. Punitive damages.
8. Incidental or consequential damages resulting from a defect covered by this Limited Warranty, including but
not limited to, loss of usage, inconvenience or damage to persons or property.

9. Damage or deterioration that occurs to the concrete as a result of improper or incorrect structural design, soil
instability or movement.
10. Damage caused by weather conditions, acts of God or acts of nature, including but not limited to the following
conditions: Wind storms, wind driven water, floods, hail, lightning, fallen trees, earthquakes, explosions, fire,
water escape, changes in underground eater table, water seepage, land settlement, sinkholes, or site conditions.
11. Items that are covered by a manufacturer's warranty.
12. Any deficiencies or non - conformities in the ready -mix concrete mixture design not formally reported to L1%1C
-itvn the one (1) year Warranty Period.
13. The use of salt, chlorides, or other chemicals on concrete surfaces will cause permanent damage and ma y result
in the need to replace such surfaces. LAIC will not be responsible for damage caused by the use of salt,
chlorides, other chemicals agents, or antiskid substances on concrete surfaces.
14. Delamination on the concrete surface due to water added during finishing, hard trowel application to
air - entrained concrete, or delayed bleed water production due to either weather conditions such as changes in
temperature or relative humidity.
LMC MAKES NO OTHER WARRANTY, EXPRESSED OR IMPLIED, WRITTEN OR ORAL, OTHER THAN
THOSE SET FORTH IN THIS LIMITED WARRANTY (INCLUDING, WITHOUT LIMITATION,
WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE OR WORKMANSHIP).
THE WARRANTIES CONTAINED HEREIN ARE PERSONAL TO THE ORIGINAL PURCHASER OF THE
SUPPLIED READY -MIX CONCRETE AND ARE NOT TRANSFERABLE OR ASSIGNABLE. NO PERSON IS
AUTHORIZED TO GIVE ANY OTHER W ARRAN TY OR ASSUME ANY OTHER LIABILITY EXCEPT BY
WRITTEN STATEMENT SIGNED BY AN OFFICER OF LMC. ORIGINAL PURCHASER OF THE SUPPLIED
READY -MIX CONCRETE DOES HEREBY EXPRESSLY WAIVE ANY OTHER WARRANTY.
The placing and finishing of the supplied ready -mix concrete are not within the control of LAIC and are, therefore, not considered
manufacturing or design defects, or ready -mix concrete mixture design deficiencies. Furthermore, this Limited Warranty will not
cover any conditions or issues arising as a consequence of eater added to LAIC supplied ready -mix concrete after the concrete has
been brought to its required slump subsequent to transit, which may adversely affect the concrete, and may prevent the concrete
from achieving the specified compressive strength, or other performance requirements stated in the Contract Documents.
Additionally, this Limited Warranty will not cover any other products added to LAIC supplied ready - xnix concrete after the
concrete has arrived on site that may adversely affect the concrete, or may prevent the concrete from achieving the specified
compressive strength, or other performance requirements stated in the Contract Documents.
LAIC's PERFORALkNCE
In the event there are any deficiencies in the mixture design of the supplied ready -mix concrete wit the one (1) year
Warranty Period, the original Purchaser shall promptly give I-2 %1C written notice of the deficiencies, and LAIC shall have
tie opportunity to inspect, remedy, replace, or repair the non - conforming concrete whose performance properties as
indicated in the original Contract Documents are adversely affected by the identified and verified concrete mixture
design deficiencies.
Failure by the original Purchaser to give LAIC written notice of non - conforming concrete, and/or the failure by tie
original Purchaser to give LIIC the opportunity to inspect, replace, repair, or remedy the said defective material, shall
constitute a waiver by the original Purchaser of any claim by the original Purchaser for the non - conforming concrete.
Any repair or replacement shall not extend the warranty term.
In the event non - conforming ready -mix concrete is to be remedied, repaired, or replaced pursuant to this Limited
Warranty, such work will be performed by LAIC or at its direction. Under no circumstances -will LMC make
reimbursement for remedy, repair, or replacement cost incurred by the Purchaser or any other person unless costs were
approved by LAIC in writing prior to incurrence.
The original Purchaser must provide 12%1C or its designee access to the Property during normal - weekday business hours
to perform its obligation under this Limited — Varranty. Failure to provide such access will relieve LAIC of its
obligations.
Acceptance of the concrete upon arrival at the point of delivery constitutes acceptance of this Limited
Warranty.

If the original Purchaser has a compliant, the original Purchaser must send a clear and specific written complaint to LAIC
prior to the time the Warranty Period expires, to the folloving address:

Limited Warranty for Ready Mix Concrete
GENERAL PROVISIONS
This Limited Ssuarranty is not effective until LIIC receives the entire sales price of the concrete supplied to the original
Purchaser. However, failure by the original Purchaser to par, or other delay in LAIC receiving, the entire sales price sliall
not extend the commencement date of the Warranty Period.
In the event that for any reason one or more of the provisions of this Limited Warranty are held to be invalid, illegal or
unenforceable, such invalidity, illegality or unenforceability shall not affect any other provision of this Limited Warranty.
This Limited Warranty shall be interpreted as if the invalid, illegal or unenforceable pros ision had never been contained
in this Limited Warranty.
It is understood that LAIC is engaged to perform services under the Contract Documents as an independent contractor
and not as an agent of the original Purchaser. The laws of the State of Texas shall govern this Limited Warranty and a
court located in Collin County, Texas must resolve any claim or dispute arising there From.

Flowable fill admixture
Description Sika Lightcrete Powder is a dry powdered surfactant packaged in a pat-
ented, ready -to -use, water soluble bag. Sika Lightcrete Powder produces
controlled low strength material (CLSM), also referred to as flowable fill,
controlled density fill (CDF), lean mix backfill, unshrinkable fill and flowable
mortar. Sika Lightcrete Powder is environmentally safe and compatible with
all conventional CLSM materials.
Application Use one 'I -lb. (454 g) bag to produce 'I cubic yard of controlled low strength
material (CLSM). Sika Lightcrete Powder should be added to the drum with
the primary mix water at the jobsite. Remove the outer bag. Add the inner
bag to the central mixer or ready mix truck drum. After all ingredients are
added, the drum should be turned at mixing speed for 5 -7 minutes.
Sika Lightcrete Powder will increase the material volume 20 % -35 %. Allow
for approximately 50% water reduction in the CLSM mix.
Controlled Low Strength Material (CLSM) can be placed directly from the
ready mix truck.

Eyes - rinse thoroughly with water for 15 minutes. Skin - wash skin with soap
and water. Remove contaminated clothing. Wash clothing before reuse.

respiration. Ingestion - Do not induce vomiting. Contact a physician.
Handling and Storage
Avoid direct contact with eyes and skin. Wash thoroughly with soap and
water after handling. Wear personal protective equipment (chemical resis-
tant gloves, goggles, and clothing). Use with adequate general and local
ventilation. In the absence of adequate ventilation, wear a properly fitted
NIOSH respirator. Remove contaminated clothing and launder before reuse.
Store product in closed container in cool, dry place to avoid contamination.
Clean Up
Ventilate area. Confine spill. Do not generate dust. Wear proper personal
protective equipment. In the absence of proper ventilation, wear a properly
fitted NIOSH respirator. Collect with absorbent material, transfer to suitable
container and dispose of in accordance with current, applicable local, state,
and federal regulations.

of product exclusive of labor or cost of labor.

SECTION 3— HAZARDS IDENTIFICATION
Emergency Overview:
Concrete is a light gray fluid mixture that poses an immediate hazard to eye tissue. Exposure of sufficient duration to
wet concrete can cause serious, potentially irreversible tissue (skin or eye) destruction in the form of chemical
(caustic) burns, including third degree burns.
If footwear or clothing becomes saturated with wet concrete, remove immediately and wash area with water and mild
soap. Do not allow prolonged contact.
Potential Health Effects:

Effects Resulting From EYE CONTACT:
Exposure to wet concrete may cause immediate or delayed irritation or inflammation. Eye contact
by splashes of wet concrete may cause effects ranging from moderate eye irritation to chemical
burns and blindness. Such exposures require immediate first aid and medical attention to prevent
significant damage to the eye.
Effects Resulting From SKIN CONTACT:
Discomfort or pain cannot be relied upon to alert a person to a hazardous skin exposure.
Consequently, the only effective means of avoiding skin injury or illness involves minimizing skin
contact, particularly contact with wet concrete. Exposed persons may not feet discomfort until
hours after the exposure has ended and significant injury has occurred.
Exposure to moist or wet concrete may cause more severe skin effects including thickening,
cracking, fissuring of the skin. Prolonged exposure can cause severe skin damage in the form of
(caustic) chemical burns.
Effects Resulting From INHALATION:
Concrete may contain amounts of crystalline silica. Prolonged exposure to respirable free
crystalline silica may aggravate other lung conditions. It may also cause delayed lung injury
including silicosis, a disabling and potentially fatal lung disease, and/or other diseases. (Also see
"Carcinogenic Potential" below. )
Respirable exposure to silica in concrete may occur only if concrete is drilled, cut, ground or
polished. Exposure to concrete dust may cause irritation to the moist mucous membranes of the
nose, throat, and upper respiratory system. The dust may also leave unpleasant deposits in the
nose.

Chronic Effects
Some individuals may exhibit an allergic response upon exposure to ready -mix concrete. The
response may appear in a variety of farms ranging from a mild rash to severe skin ulcers. Persons
already sensitized may react to their first contact with the product. Others may first experience this
effect after years of contact with ready -mix concrete. Minimizing contact with skin is the basic
protection to reduce this exposure..

Carcinogenic Potential
Ready Mix Concrete is not listed as a carcinogen by NTP, OSHA, or IARC. It may, however,
contain trace amounts of substances listed as carcinogens by those organizations.
Crystalline silica, a potential trace level contaminant in portland cement, is found in the aggregate
components in varying percentages and is classified by IRAC as a known human carcinogen
(Group 1). NTP has characterized respirable silica as "reasonably anticipated to be [a] carcinogen ".

Immediately flush eyes thoroughly with water. Continue flushing for 15 minutes, including under the
lids, for at feast 15 minutes_ Seek medical attention immediately.
Wash skin with water and pH neutral soap or mild detergent intended for use on skin.
If clothing or footwear is saturated remove immediately and wash area with water and mild soap. if
contact has been severe enough to cause reddening or actual burns to skin, place sterile bandage
on area and seek medical attention.

In wet form, concrete cannot be inhaled.
Ingestion
In wet form, concrete is unlikely to be ingested. If concrete enters mouth, wash out with water
immediately. Seek medical attention if any burning sensation or actual burns occur.

SECTION 6 — STABILITY AND REACTIVITY
Stability: Stable
Incompatibility: Portland cement reacts with water to produce a caustic solution,. pH 12 to pH 13.
Wet concrete is alkaline. As such it is incompatible with acids, ammonium salts,
and aluminum metal. Aluminum powder and other alkali and alkaline earth
elements will react in wet mortar or concrete, liberating hydrogen gas. Concrete
dissolves in hydrofluoric acid producing corrosive silicon tetrafluoride gas.
Silicates react with powerful oxidizers such as fluorine, chlorine, trifluorides, and
oxygen difluoride.
Hazardous Decomposition: Will not occur.
Hazardous Polymerization: Will not occur.
SECTION 7 - HANDLING AND STORAGE
Normal temperatures and pressures do not affect the material.
Promptly remove dusty clothing or clothing which is wet with concrete fluids and launder before reuse. Wash
thoroughly alter exposure to dust or wet cement mixtures or fluids.
Ready Mix Concrete is premixed at a plant or in a truck mixer drum and delivered to the end user in semi -fluid state
ready to be placed to set in final form.
SECTION 8 EXPOSURE CONTROLSIPERSONAL PROTECTION
Eye Protection:
Safety glasses with side shields, or goggles, should be worn when engaged in activities where cement dust, wet
cement, or concrete could contact the eye. In extremely dusty environments and unpredictable environments,
wear unvented or indirectly vented goggles to avoid eye irritation or injury. Contact lenses should not be worn
when working with ready mix concrete or fresh concrete products.
Skin Protection:
Prevention is essential to avoid potentially severe skin injury. Avoid contact with unhardened (wet) concrete
products. If contact occurs, promptly wash affected area with soap and water. Wear impervious clothing and
gloves to eliminate skin contact where prolonged exposure to unhardened ready mix concrete products might
occur. Wear boots that are impervious to water to eliminate foot and ankle exposure. If standing in wet concrete
rubber boots must be worn to prevent injury
Wet concrete may splash into open boot tops and saturate socks and remain in contact for a lengthy period of
time. Prevention is to ensure that boots are fully laced up.
Do not rely on barrier creams; barrier creams should not be used in place of gloves.
Periodically wash areas in contact with dry Portland cement, wet cement, or concrete fluids with a pH- neutral
soap. Wash again at the end of workday. If irritation occurs, immediately wash the affected area and seek
treatment. Clothing saturated with wet concrete should be removed immediately and replaced with clean, dry
clothing. too not allow clothing saturated with wet concrete to remain in contact with skin for any period of time.

Respiratory Protection:
Avoid actions that cause dust to become airborne. Use local or general ventilation to control exposures to below
applicable exposure limits.
Use NIOSH/MSHA,- approved (under 30 CFR 11) or NIOSH- approved (under 42 CFR 84 after July 10, 1998)
respirators in poorly ventilated areas, if an applicable exposure limit is exceeded, or when dust causes discomfort
or irritation.
