I met with Larry Davis on site and Larry informed me that he wanted a report from JRJ Safety person and if he was not satisfied, he would contact OSHA. I informed him that JRJ Safety was heading to the site and would arrive at 3pm to see what work was being questioned. I observed the men in the basket, with harnesses on and they were strapped to the strap holding up the basket. JRJ Safety arrived on site at 3:05pm and observed work being performed and made some minor adjustments. JRJ Safety also spoke with Dennis Troutz, N TX Bridge, me, and yourself once you arrived on site regarding the work being performed. JRJ Safety informed yourself that he would be sending you something also. We are all striving to complete this project and safety is first and foremost. If there is an issue on the site I would appreciate the chance to meet with parties and see what the issue is and correct it without the threat of someone telling me and my subcontractor they were calling OSHA. If there is anything else we need to discuss regarding this, please feel free to contact me. I am on this site several times a day and when I am unable to be on site, I have spoken with Larry about when I would be. Project Engineer (972) 304-3681 December 9, 2010 SAFETY REPORT Per your request, Cornerstone Safety is forwarding the following Safety Report. On 12/9/2010, at approximately 1:45 pm, I was contacted by Dean Dumke, Project Manager for JRJ Paving Company concerning Safety issues at the Bethel Road jobsite. upon arriving on the jobsite at approximately 3:05 pm, I met with Dean Dumke of JRJ Paving and Dennis Troutz with North Texas Bridge Co. Inc. During this meeting Keith Marvin with the City of Coppell arrived on site. All jobsite recommendations were discussed and contractor was able to resume work. North Texas Bridge Co. Inc. subcontractor performing finishing texture work on the outer perimeter of the bridge on Bethel Road. The contractor is using a Sky Trac to suspend employees over the outer perimeter to perform work. The contractor was not observed in violation of any OSHA rules and regulations or in violations of any JRJ policy and procedures. However, the contractor was instructed to secure the OSHA approved straps to the Sky Trac to ensure stability of personnel basket. North Texas Bridge Co. Inc. was instructed to provide daily inspections of Sky Trac by a competent person. The contractor was also instructed that all OSHA approved straps should be inspected daily before suspending the personnel basket over the outer perimeter of the bridge. All employees working from the personnel basket are required to use personal fall arrest systems, and attach to personnel basket. A competent person shall be present on site at all times. As Safety Coordinator of JRJ Paving we appreciate the opportunity to provide quality construction services now and into the future with the City of Coppell. We assure every effort to develop and maintain a safety jobsite for all JRJ employees and all subcontractors.
