GME senior geotechnical engineer, Doug Ziolkowski, traveled to the job site to observe the beginning of the pier
drilling operations. The GME engineer arrived on -site at 7:45 a.m. Upon arrival, the GME engineer met with
GME field representative, Bill Harvey, who will be performing the pier observations services for GME. The
GME engineer reviewed the project geotechnical report by GME to familiarize the field representative with
the soil and groundwater conditions anticipated to be encountered. GME did not have any foundation plans;
so, the GME engineer borrowed the concrete subs plan set to review and familiarize the GME field representative
with the structural aspects of the project. GME made a field sketch of the pier layout plan.
At arrival, the GME engineer found that none of the pier locations had been marked on the ground. The concrete
subcontactor and his personnel completed layout at 9:15 a.m. Pier drilling by Dallas Foundation began at 9:25 a.m.
at Piers C -1 and B -1. Both piers were drilled to 8.3 feet depth and were dry and relatively clean at completion.
After observing the 1 pier job, the GME engineer felt confident the GME field representative was familiar with
the job requirements and left the site.

A GME representative, Bill Harvey, arrived at the job site at 7:40 a.m. to observe pier installations. Initially,
Mr. Harvey met with GME senior engineer, Doug Ziolkowski, to review pier layout, soil conditions and
installation details and procedures. Pier layout was completed at 9:15 a.m. and drilling began at 9:20 a.m.
A total of 16 -48" diameter strait t -shaft siers were completed in au:ered holes that were .lumb and
reasonably clean and dry prior to concrete placement. Rebar cages were fabricated in general compliance with
the design plans, and concrete was placed that appeared to be in general compliance with the design specifications.
A total of 54 cubic yards of concrete was placed. A concrete slump test was performed, and one set of four
concrete test cylinders was cast for strength verification. All pier work was completed at 4:10 p.m. GME
representative left the job site at 4:15 p.m.
