Notice of Contract Termination
Dear Mr. Ziolkowski:
This letter will serve as written notice that your current contract will be terminated 30
calendar days from the date of this letter as pursuant to Section 10 of the contract. GME
Consulting Services, Inc. has failed to perform to the level for which they were contracted.
The contract will be terminated June 22 2011 due to failure to attend scheduled meetings,
lack of progress based on the anticipated schedule, and unprofessional conduct of the drilling
crew. The professionalism and performance of work to date has not met the expectations of
the City of Coppell. As a contractor working in coordination with the Engineering & Public
Works Department you are viewed by the public as an extension of this department and the
City. Therefore, GME's level of service is unacceptable.
As of the date you receive this letter, do not perform any further drilling or testing work.
Please deliver the eight (8) un- tested, preserved soil samples & boring logs to the City of
Coppell's Service Center at 816 S. Coppell Rd on Tuesday May 24 2011. At your earliest
convenience turn in a final invoice to this office for all completed work. If you have any
further questions, please contact me directly.
