We are pleased to inform you that the referenced facility has been inspected and found to be in substantial
compliance with provisions of the Texas Government Code, Chapter 469.
The inspection results will be forwarded to the Texas Department of Licensing and Regulation for issuance of
the final approval letter. For newly constructed buildings and facilities, the Department will provide a Notice of
Substantial Compliance (Certificate and Decal) to the owner upon receipt of a completed Notice of Substantial
Compliance Request Form.
Please note, this determination does not address the requirements of the Americans with Disabilities Act
(ADA), (P.L. 101 -336), or any other state, local, or federal requirements. For information on the ADA, please
