Final Inspection of Change Order Number 1 & 2 -~ Sandy Lake Road Water Line
A final inspection for the work associated with Change Order Number 1 and 2 to your contract with the City of
Coppell for the construction of the Wagon Wheel Ranch Elevated Storage Tank was conducted today (August
19, 1998). Change Orders 1 and 2 cover the work required to construct the Sandy Lake Road Water Line from
Coppell Road to Whispering Hills Drive. The following were in attendance:
Larry Davis, City of Coppell Brad Lands, Mike Albert Construction
Joe Swinnea, Landmark Structures Vance ????, Mike Albert Construction
Jim ????, Mike Albert Construction Gary Hendricks, Shimek, Jacobs & Finklea, L.L.P.
The following items were found in need of correction or repair:
1. Coppell Road Intersection: Repair or replace damaged traffic control buttons in the intersection of
Coppell Road and Sandy Lake Road.
2. Paint all valve covers blue and mark their location on the adjacent pavement with a painted blue "V".
3. Replace the broken concrete valve pads at Forest Cove Drive and Whispering Hills Drive.
4. Route and seal (silicone) the concrete pavement repair joints in Whispering Hills Drive.

As we discussed, either a separate maintenance bond or a maintenance bond for partial completion of the project
will be required prior to final payment being made on this portion of the project. Once these punch list items are
completed to the satisfaction of the City and the necessary maintenance bond is in hand, we will recommend
payment and release ofretainage for change orders 1 and 2.
In addition to the corrected items, Brad Lands provided us with information concerning the field changes made
during the construction of the project. We will make the necessary changes to the construction record drawings
and provide the City with the required number of sets.
