Box 540787 Dallas, TX 75354-0787 Re: Change Order No. Six (6) -Lighting, Landscaping & Irrigation Belt Line Road Improvements - Coppell, TX Dear Mr. Albert: As we discussed in our pre-construction meeting on January 5, 1989, you are hereby issued a Notice to Proceed with Construction on or before January 16, 1989 for the above referenced Change Order No. six (6) items of work. Eighty-four (84) calendar days are allotted for this work. Bad weather days, as defined per the contract, are not included in this schedule and will be accounted for additionally by crediting Ed Bell Construction Co. for those additional days. Weather permitting, the contract completion date for this work shall be April 9, 1989. All the terms of the Belt Line Road Improvement contract shall continue to be in effect until final acceptance of the work by the City of Coppell. In addition to the Change Order No. Six (6) items of work, the metal beam guardrail and decorative concrete rail shall be required to be installed and completed by April 9, 1989, also. A copy of the fully executed Change Order shall be provided to you on January 16, 1989. Below is a brief outline of the items we discussed and/or agreed to at the meeting. Attached is a copy of those in attendance at the meeting. Irrigation System and Light Foundations shall commence construction first. Construction will start at IH635 and proceed North in the median. Landscape will commence upon near completion of #1 above. During.the median construction (#1), one inside lane each direction shall be closed to traffic, during daytime only, planned in 5 increments or stages to accomplish the project length. Proper barricades, lane transitions and advance warning signs identifying the construction zone(s) shall be provided for the duration of the project and shall conform to the Texas Uniform Traffic Control Manual for Construction Areas. Lane closures shall be reopened at the end of each work day and weekends. Left turn lanes shall be preserved even during regular inside lane closures. Along Northlake, the North bound outside lane shall be closed to accommodate guardrail and landscape construction. Coordination with the median work will be scheduled to minimize or prevent more than one lane being dlosed at any one time. Barricades shall also adhere to paragraph #6 above. Regular weekly meetings shall be held at the Cowboy/Belt Line Road intersection at 1:00 pm each Thursday afternoon to discuss progress, schedules, payment applications, etc. Payment applications shall be submitted to Mr. Randy Fleming, Ginn, Inc., for review and processing by the 10th of each month for work completed through the last day of the prior month. Use present application forms and continue numbering system in effect. Attach additional sheets for extra work or Change Order No. six (6) items if necessary. No payment shall be made for materials on hand for Change Order No. six (6) items. However, this shall not be applicable to the guardrail materials. Ail electrical work shall be done by a licensed electrician holding a permit to do work within the City of Coppell. The city Building Inspection Department must "green tag" all electrical work prior to acceptance and TU Electric providing any power. TU Electric shall identify in the field which light foundations shall receive power source feeds. At these locations, TU Electric requires 3, not 2, PVC sweeps to be installed with the foundation. TU Electric will provide the PVC material for Ed Bell Construction Co. to install. Ginn, Inc. shall require to be provided with 6 sets of all submittals and shop drawings and Ms. Linda Smith, ASLA shall require 1 set, with the exception of concrete mix designs for the light foundations, which only need to be submitted to Ginn, Inc. 14. city (Parks Dept.) needs 3 quick coupler keys provided at completion for access to the irrigation controllers. 15. Ail water valves shall be operated by the City of Coppell Public Works Department only. Contact Mr. Howard Pafford, 462-8495 for assistance. 16. Ail water needed to accommodate Change Order No. six (6) shall be provided by the City at no charge to Ed Bell Construction Co. Usage, however, shall be required to be metered. Notify Mr. Pafford for both coordination and identification of an acceptable source location. Ed Bell Construction Co. must inform the Fire Department at telephone #911 if any fire hydrants are to be out of service during this time. 17. Ginn, Inc. and TU Electric shall coordinate ahd advise Ed Bell Construction Co. as to where the irrigation power sources are to be located and will request unit prices for this field change required. In closing, in the situation of any conflicts between the above items and the Belt Line Road Contract Documents and Specifications, the latter shall govern and overrule for clarification and intent purposes.
