Dear Sirs:
This letter is to inform you that I have been retained by
Sharon Wilkinson to represent her in a claim for damages arising
out of an accident on the above date. My investigation of this
incident leads me to believe that the negligence of the City of
Coppell and its employees and/or agents was the sole proximate
cause of my client's injuries and resulting damages.
Enclosed please find a signed, completed claim form which
was obtained from your office. Attached to the claim form are
copies of medical bills that have been incurred to date.
If you are protected by liability insurance, please forward
this letter directly to the insurance carrier. If you are self-
insured, please contact me at your earliest convenience to
discuss this claim.
Please be advised that if I have not heard from you within
thirty (30) days of the date of this letter, I will file suit
immediately to protect the interests of my client.

walked down the alley :o see how my son ~as comin~ alou~ tryfng
-~'-~ let the truck out of mud at Hoore Road and our alley. I
walked alon8 side of the truck on the passenEer side and stepped
in:o deep mud ~i:h my right foo~. I tried to pull my foot out
and when I did my boot came right off. I stepped onto my lef~
foot and ~visted i: badly; then I fell onto my right leg and
dislocated my foot and broke both bones in my ankle.
