Ms. Davis,
Please print the attached document "SW0801-ES100713.pfd" & the "w-9 form,pdf". On the second sheet of the
letter, please sign it and then fill out the w-9 form. Then either scan it in or mail it back to my office at 255
Parkway, Coppell, TX 75019.
Once we have all three owner's signatures we will then process the checks and it will take approximately 2
weeks. When the checks are ready I will call you all and have you come into our office to sign the easement
document and give you the checks.

Mr. Gaddy & Mr. Gaddy,
If you find the easement amount acceptable please execute the letter & fill out a W-9 form for each of you.
Also, please pass this information on to Liz Davis for her execution as well. Once we have the letter & w-9 form
we can then process the checks. When the checks are in our office we will call you and you each will need to
come to our office and we can have the easement document signed and pass the check on to you.

find the offer acceptable. This offer is based on keeping the sidewalk route as shown on the exhibit attached
and leaving the area between the curb and sidewalk as grass with maintenance of the grass your responsibility.

Please review the attached, revised sidewalk easement documents. I was able to track down all of the
ownership information on this property and attached them as well. Please note that the exhibit B where it shows
the ownership, I am in the process of having our surveyor revise this exhibit to call the correct ownership.
Please try to get Mrs Hunnicutt to sign the document. If you have any questions, please feel free to contact
me. Also, if needed we have a notary in our office.
