ITEM: S-1113, Church of the Apostles, Evisco_tnd, zoning change from SF-
7 (Single Family-7) to SF-7-S.U.P. (Single Family-7, Special Use
Permit) to allow the construction of a church on 7 acres of
property, located at the southeast comer of MacArthur Blvd. and
Starleaf Street, at the request of Arthur Weinman Architects.

Master Site Plan
1) A left turn lane and median opening should be constructed along with the Phase II
development.
2) The Carrollton-Farmers Branch School District should be approached about obtaining a
private access easement from the north property line of the church to the south right-of-way
line of future Starleaf Street for a second point of access in the future.
3) To ensure the City can access our sewer line in the future, no improvements should be
constructed in the sewer easement. This includes retaining walls and monument signs.
Site Plan
1) 6" thick 3000 psi reinforced concrete on a lime stabilized base is required in the fire lane and
5" 3000 psi on a lime stabilized base in the parking areas. Remove reference to 4" concrete.
2) Brick paver band should be 8' wide and one foot inside the property line.
3) Remove monument sign from existing sewer easement.

1) Show finished floor elevation of the building.
2) Show all easements. This includes the existing sanitary sewer with volume and page number
and any other easements necessary for the water meters and vaults.

Grading and Utili~_ Plan
1) Minimum water line size for fire protection is 6".
2) Provide meter vault for fire line.
3) Site plan shows f'tre hydrant at southeast comer of church, however the plan does not show
a fire line to the hydrant. All fire hydram locations and line sizes are to be approved by the
Fire Marshall.
Landscape Plan
1) No trees are allowed in the visibility easements.
