ST0501-CS120419 T H E G I T Y O F COPPELL * Agit; A 5 , 8 April 19, 2012 Robert Caudill Tiseo Paving Company RE: Deforest Rd, Project #ST 05 - 01 Dear Mr. Caudill: This is a letter of concern regarding work on the Deforest Road project, ST 05 -01. The City of Coppell has several concerns and we feel the project's progress and completion may be in jeopardy. Some of the concerns are related to construction practices and day -to -day operations onsite. Some examples are listed below, but are not intended to be a complete list or to interfere with project timelines. We have had concerns for some time now related to the retaining wall construction along the south side of Deforest Rd. As you are aware, on more than one occasion re- inspection and re- scheduling of inspection personnel has been required due to a failure to comply with City of Coppell construction standards and /or misapplication of the materials, specifically steel inspections. Each re- inspection puts the project off by another activity work day. Remember, this project has a bonus /penalty timeline aspect written into the contract. Other areas of the project are also of concern related to safety — for project personnel and for citizens. Two items in particular should be improved upon: 1) Maintain in good operating order and proper location any and all traffic control devices 2) Erosion control devices should be in place and operating at all times. Due to work activities, these devices at times have been removed or displaced, and on a number of occasions we have requested that Tiseo make necessary corrections to maintain proper use of the devices. Please be advised there are other concerns on the progress of the project. This letter is in no way intended as an instrument to identify every day -to -day concern, but it is designed to place notice of some major concerns offic ially.
