CHANGE ORDER NO.: Four (4) DATE: April 30, 2012
1. This change order modifies the joint trench installation of the conduit for Franchise
Utilities. New pay items are added for secondary hand holes and pipe stub-ups.
2. This change order modifies the parking lane striping from 3-M Staymark to thermoplastic.
3. This change adds a pay item for a segmental block retaining wall to be built in front of the
old Minyard Store adjacent to the project. This wall is necessary as a result of the grade
changes associated with this project.
4. Finally this change revises the electrical plans by replacing the rack mounted power
service with a black powder coated pedestal, and includes a timer and photocell for the
tree well receptacles.
