I will present the raw data to you along with my analysis for each location. Attached for reference was URETEK's original proposal from September 12, 2011 for work within the 30" Water Line Trench only. • We had estimated 200,000 pounds for the 2 miles of the 30" Water Line Trench (8' wide). • We had proposed a pilot project to stabilize 250 Linear Feet by 8' wide (2000 SF) of the 30" Water Line Trench for 5000 pounds of material. • This was estimated at 20 lbs per Linear Foot, or 2.5 lbs per Square Foot. After much discussion, it was decided to proceed with a pilot project with four locations and 7000 pounds. The four locations were selected in an attempt to cover the various problems that exist under the roadway and gauge the material consumption per location. All four locations were full width (one direction) of the roadway. As you might recall, my hope and optimistic projection was to be able to complete all the locations for only 5000 pounds, but felt better about having up to 7000 pounds to complete the entire pilot project scope. We pumped a total of 7065 pounds to complete all four locations (we charged only the 7000 pounds per the PO—but pumped the remaining 65 pounds required at no charge). • Location 1 WB included the 30" Water Pipe Trench and had significant settlement, significant voids as evidenced by the vibration felt as traffic crossed over the panels, and damage to the concrete panels (36' L x 35' W, 1260 SF). • Location 1 EB was located directly across from Location 1 WB (44' L x 25' W, 1100 SF). • Location 2 EB included a section having problems due to a leak at the connection to the catch basin (32' L x 25' W, 800 SF). • Location 3 EB included a section with no new utilities added that appeared to be in good shape with little or no problems (24' L x 25' W, 600 SF). • All four locations totaled 3760 SF. • Injections totaled 7065 pounds or 1.9 lbs/SF. LOCTATION 1 West Bound • Total Area injected was 36' L x 35' W (1260 SF). • We injected 1602 pounds or about 1.3 lbs/SF at-3.5' on a 4' x 4' grid pattern. Each injection averaged about 20 pounds. • The 30" Water Pipe Trench is located within this area. In addition to the injections at-3.5', we injected at-5' directly over the pipe and then 3'on either side of the pipe we injected at-7'. The additional injections in this area totaled 1154 pounds. Each injection averaged about 43 pounds each. • The total material injected for this location was 2756 pounds for an average of 2.2 pounds per SF. • In an attempt to look only at the 30" Water Pipe Trench area, I have selected the data from columns D & E at-3.5' for a total of 350 pounds (average of 20 pounds each). Adding that to the 1154 pounds injected at-7', -5', and -7' around the pipe yield a total of 1504 pounds for the 36' L x 8' W(288 SF)trench area. This yields an average of 5.2 lbs/SF for the trench area. This is just more than double what we had estimated in our original proposal for an average over the entire trench (good and bad areas). • If all the areas over the trench and within the trench were as bad as this area (perhaps the worst), then the estimate for the 2 miles of stabilization of the trench (8' wide) would have to be revised to 5.2 lbs/SF or 440,000 pounds. • However, we did in fact choose the worst area, so we expect there to be some savings in the other areas, perhaps dropping the projection for the 2 miles of trench stabilization to 300,000 pounds. That being said, I do not believe it is reasonable to expect it to be around the original estimate of 200,000 pounds for the entire 2 miles of 8' wide trench. LOCTATION 1 East Bound • Total Area injected was 44' L x 25' W (1100 SF) • We injected 1425 pounds or about 1.3 lbs/SF at-3.5' on a 4'x 4'grid pattern. • The average of 20 pounds per injection is roughly the same as the injections at-3.5' for Location 1 West Bound. LOCTATION 2 East Bound • Total Area injected was 32' L x 25' W (800 SF) • We injected 1474 pounds or about 1.8 lbs/SF at-4' on a 4' x 4'grid pattern. • The average of 30 pounds per injection is 50% more than what we saw at-3.5' at Location 1 WB & EB. This is because the outside lane near the catch basin averaged 40 pounds per injection while the inside lane by the median required the same 20 pounds per injection as Location 1 WB & EB. • Clearly the leak at the catch basin (under the outside lane) allowed significant amount of soil to escape from under the pavement, causing significantly weaker soils than in the other lanes. LOCTATION 3 East Bound • Total Area injected was 24' L x 25' W (600 SF). • We injected 1413 pounds or about 2.4 lbs/SF at-4' on a 4' x 4' grid pattern. • The average of 40 pounds per injection is double what we saw at -3.5' at Location 1 WB & EB. • The DCP tests showed better results in this area (as expected)than in locations 1 WB and EB & location 2 EB. • At this time I do not understand why this area took significantly more material than expected. I don't know if this was just an anomaly and is data that should be considered an outlier or if we need to learn more about what else is going on here. OVERALL: • Total of 7065 pounds for 3760 SF—approximately 1.9 lbs/SF. Ed will be bringing this report to you to discuss with you this week. PO Box 1929 URETEK USA, Inc. is pleased to present this proposal of work to solve the stabilization Tomball TX issues that you have due to un-compacted soils around and above a water line. 77377-1929 13900 Humble Rd PROBLEM: Tomball TX 77377 The soils in the water main ditch line were not compacted properly and/or have loosened. T:888-287-3835 Un-compacted soils can cause settlement and/or collapse of the pcc roadway above the ditch T:281-351-7800 F:281-351-0884 line. The URETEK MethodTM PROPOSED SOLUTION: Deep Injection TM Stitch-In-Time TM URETEK will use the patented URETEK Deep Injection Method and our patented URETEK 486 STAR material to void fill and stabilize the repair areas. In accordance with the attached specification, URETEK will perform the following operations: 1) Work schedule to complete the entire project is estimated at 100 work days with one truck and crew or 50 days with two trucks and crews. 2) Profile roadway 3) Drill injection holes on approximately 2' centers transverse to the pipe, 4' apart along the length of the pipe; install injection tubes through the holes to the planned elevations at -5.5' directly above the pipe and then at-3.5' and at -7' on either side of the pipe(2' off centerline of the pipe). 4) Adjacent holes are to be left open during injection as exit ports to allow for water within the soils to escape from under the roadway. 5) Inject the highly expansive, high strength, lightweight, hydro-insensitive URETEK 486 STAR material through the tubes and into the soils to fill the voids, stabilize the soils to a planned depths of-5.5' over pipe and then at -3.5' and -7' on each side of pipe. Injection will continue until lmm of movement is detected at the surface of the roadway confirming adequate soil stabilization. Then continuing injections until roadway is lifted to proper elevation and grade. Note that this material is hydro- insensitive and can be injected into wet soil conditions without compromising the integrity of the material. 6) Drill out top 2"of grout injection holes and fill with a non-shrink cementitious grout. 7) Upon approval of repair work, utility locates will be ordered. When locates are complete repair work will be coordinated between Mr. Per Birdsall and Uretek Operations. ESTIMATED MATERIAL USAGE: 1) Entire project consisting of 2 miles of roadway is estimated at 200,000 lbs. Based upon the use of SMART BUY of$5.50 per lb for total project cost of $1,100,000. (Without Smart Buy the price is $6.00 per pound), utilizing the pattern of injection stated above. Area to be affected is about 3 to 4 feet on either side of the pipe or 6 to 8 feet width by 2 miles long. The project can be broken into whatever sections as needed to accommodate your budget. 2) We recommend doing a small test section to will provide a more accurate estimate of the expected material consumption required for the entire project. This test section can be any size, but a 5000 pound ($27,500)test for approximately 250 linear feet of roadway is recommended. We can perform whatever size test you prefer or can budget. 3) The pricing on Smart Buy is $5.50 for minimum order of 10,000 pounds. We will waive this minimum for any sized test section for this project. 4) If you have any questions about Smart Buy, please don't hesitate to ask. If we can't answer the questions directly, we will put you in touch with the Texas Comptroller's office in Austin PROPOSED COST: URETEK is pleased to offer SMART BUY pricing at $5.50 lb. Client will only be invoiced for material used. In the event more material is needed to complete repair, work will not continue to until written approval is received from client. We are local so mobilization will be waived. Saw cutting not included and not required for this project. SCHEDULE: URETEK should be able to complete 100 to 150 linear feet per truck per shift. For the entire project that would be 70 to 100 truck shifts. Depending on the need and the scope, we can of course put multiple trucks on the project. Schedule will be discussed between URETEK Operations manager and owner after all paperwork is in order. Due to the hydro-insensitive nature of the URETEK 486 STAR material, URETEK can continue to work and inject the material in the rain. We would stop the project if there was lightning or if the rain posed a traffic safety threat to our workers or to the traveling public. NOT INCLUDED: 1) Bond—add 2.5%if bond required. 2) Saw cutting 3) Traffic Control WARRANTY: URETEK will provide a two-year bumper-to-bumper warranty against movement of more than '/4" of the injected area. In the unlikely event that movement of more than '/4" in the injected areas occurs,URETEK will return to inject the affected area to lift to proper grade at. no cost to the owner. SIMILAR PROJECTS as REFERENCES: See attached NAVFAC press release and photos of our research project for the Navy conducted in 2007 & 2008. We injected our material into a loose un-compacted fill of rock and soil and successfully stabilized the soils to the satisfaction of the DOD (even under severe wet soil conditions in Florida just after a hurricane passed through). Another project of significance was completed in 1999. We injected down to 14' under the airfield pavement at Andrew's Air Force Base where Air Force One lands. Recent reports from 2007 and 2008 indicate that the pavement has not moved and our process is deemed successful — having now been in place for 10 years. Dr. Brown (Vice President for Engineering — URETEK USA) was then a consultant for the DOD and was tasked to bring the URETEK DEEP INJECTION Technology to the DOD and oversaw the successful project. MERIT SHOP CONTRACTOR: General contractor recognizes that URETEK is a merit shop contractor, and all services provided by this sub-contractor agreement will be on a merit shop basis. All reference to labor agreement of any kind, or alluded to, in the principle contract or the sub-contract, are set aside, and not part of this sub-contract. SAFETY: Safety—Our people are our most important asset and safety is our top concern. URETEK has an impressive safety record. URETEK's Workman's Comp discount rate is 0.69 due to our experienced crews and our safety record. URETEK has an extensive safety manual directly pertaining to our polyurethane injection process. URETEK has employed an industrial hygienist company to produce a safety report on our production unit, process, and personnel.
