Dear Mr. Marshall:
In accordance with the November 7,2012 request of Mr.Larry Davis,we are pleased to submit the
following estimate of materials testing and observation services necessary for completion of the
above referenced project. The estimate is based on a construction schedule provided by Mr.Walter
Mitchell of Tiseo Paving and typical past testing frequencies.
Including our October 22,2012 invoice,thus far we have billed$39,665.85. Our original contract
estimate was for$39,996.00..We estimate the fees for project completion beyond our October 22,
2012 invoice will be as follows:

Based on testing provided since October 22,2012,anticipated remaining schedule information from
Tiseo Paving and Mr. Larry Davis our total estimated fee to perform the aforementioned tests and
services is detailed above.
It should be noted that this proposal is based on the estimated number of tests and quantities
mentioned in this proposal and not a lump sum price. The construction sequence or job conditions
may require more or fewer testing or observation. Only work which is actually performed will be
charged for and all charges will be in accordance with the corresponding unit fees quoted in this
proposal or our Schedule of Fees.
If these additional services are acceptable please sign the enclosed copy and return it to us at your
earliest convenience.
