Contract Amendment #1
Dear Ms. Baskins:
This Amendment No. 1 is for additional design survey and engineering services for project scope
modifications to the above referenced project. The additional scope for this project includes the design of
a traffic signal at the intersection of MacArthur Blvd. and Deforest Road. The additional survey is to
include the east side of MacArthur Blvd at DeForest Road which was not previously included in the scope
of the project.
The additional work is outlined below:
Preparation of plans and specifications for the installation
of a Traffic Signal at the intersection of MacArthur Blvd
and DeForest Rd. to be provided by Lee Engineering.: $12,000.00
Additional Survey for the east side of MacArthur Blvd at
the intersection of MacArthur Blvd. and DeForest Rd.: $580.00
Property Research and ROW determination near the
intersection of MacArthur Blvd. and DeForest Rd.: $675.00
TOTAL: $13,255.00
