4. Inject the following test sections listed below. All locations are planned for full panel
injections and injections in the trench alongside the pipe as originally planned — no
partial panels.
5. Location 1 — all three water lines involved. Both sides of the street — full width.
North Side — 36' long x 35' wide — centered on the air relief valve system. South
Side — 44' long x 25' wide
6. Location 2 — Drain box & Storm Drain Line. South Side only — 30' long x 25' wide
— full width.
7. Location 3 — Good pavement — no relocation of utilities was involved. South Side
only — 21' long x 25' wide — full width.
8. Drill out top 2" of grout injection holes and fill with a non - shrink cementitious grout.
9. Collect data on material usage in each injection point to facilitate best possible
material estimate for each type of problem going forward.
ESTIMATED MATERIAL USAGE:
This project consisting of the 3 test sections of roadway is estimated at a maximum of 7,000
pounds. If project does not require the full 7000 pounds, we will extend the test sections
accordingly to provide URETEK and City of Coppell more information regarding the
estimated needs for a full project.

PROPOSED SOLUTION:
Stitch -In -Time®
URETEK will use the patented URETEK Deep Injection Method and our patented URETEK
486 STAR material to void fill and stabilize the three repair areas as noted on the two
attached drawings (and as marked in orange paint on the curb with arrows pointing back at
each other).
In accordance with the attached specification, URETEK will perform the following

PROPOSED COST:
URETEK is pleased to offer SMART BUY pricing at $5.50 lb for a purchase order issued
through the SMART BUY system. Client will only be invoiced for material used. Saw
cutting not included and not required for this project.
SCHEDULE:
The project should last approximately I week. Tentative start date is 3/12/2012 as traffic
should be lighter during the school's spring break. Due to the hydro- insensitive nature of the
URETEK 486 STAR material, URETEK can continue to work and inject the material in the
rain. We would stop the project if there was lightning or if the rain posed a traffic safety
threat to our workers or to the traveling public.
NOT INCLUDED:
1. Bond
2. Saw cutting
3. Traffic Control
WARRANTY:
URETEK will provide a two -year unconditional warranty against movement of more than '/4"
of the injected area. In the unlikely event that movement of more than '/4" in the injected
areas occurs, URETEK will return to inject the affected area to lift to proper grade at no cost
to the owner.
MERIT SHOP CONTRACTOR:
General contractor recognizes that URETEK is a merit shop contractor, and all services
provided by this sub - contractor agreement will be on a merit shop basis. All reference to
labor agreement of any kind, or alluded to, in the principle contract or the sub - contract, are
set aside, and not part of this sub - contract.
INDEMNIFICATION and HOLD HARMLESS:
To the fullest extent permitted by law, URETEK will indemnify and hold harmless the
Owner, it's consultants, agents, and employees from and against claims, damages, losses and
expenses, including, but not limited to attorney's fees, arising out of or resulting from
performance of the Work by URETEK, provided that such claim, damage, loss or expense is
attributable to bodily injury, sickness, disease or death, or injury to or destruction of tangible
property (other than the Work itself) including loss of use resulting there from, but only to the
extent caused in whole or in part by negligent acts or omissions of URETEK, it's
Subcontractors, or anyone directly or indirectly employed by them.
SAFETY:
Safety — Our people are our most important asset and safety is our top concern. URETEK
has an impressive safety record. URETEK's Workman's Comp discount rate is 0.69 due to
our experienced crews and our safety record. URETEK has an extensive safety manual
directly pertaining to our polyurethane injection process. URETEK has employed an
industrial hygienist company to produce a safety report on our production unit, process, and
personnel. The report clearly shows on -site safety procedures in using and handling the
process chemicals and equipment. The report is available upon request.
Utility Locates will of course be called in by URETEK USA so that all utilities are marked in
the locations to be worked. However, since we need to stabilize in, around, and above the
trench areas that contain the water pipes, we will need to drill holes within the "safety" zone
nonnally considered a "no -dig" area. After we drill through the concrete above the water
pipe, we will vibrate our injection tubes down around and above the pipe utilizing a hilti drill.
The injection tubes are steel with a steel tip on the end to prevent soils from entering the tube.
While this should not pose any threat to the water pipe, no doubt a representative of the utility
company will need to be on site during this process. We may need your assistance in
arranging this with the water company. We may also need to have a pre - construction
meeting with you, the utility company and URETEK to discuss all aspects of the project.
INSURANCE:
URETEK has insurance in excess of any requirements client may have. URETEK also
carries a $20 million umbrella policy.
