Per your r%ua~/, ~ Final Inspection was performed on the above
referenced project on June 28, 1990. The following is a list of
those items which were noted as incomplete, incorrect, or in need
of touch-up:
HVAC-STOREROOM
1) Valve on cold water of wash sink is loose and slips when
turned.
OPERATOR,S ROOM
1) Aluminum window trim (to pump room) should be squared at the
corners.
2) Dust mini-blinds and clean glass windows.
3) HVAC thermostat is loose and crooked.
4) Touch up paint on door frames (all three doors) where
caulking was done.
5) Door knob to restroom is nicked and the finish is peeling -
replace knob.
6) Caulk bottom of all door frames and all tile cuts.
7) Clean and buff vinyl tile floor.
8) Clean debris from light fixtures.
REBTROOM
1) Shower light and vanity light do not work.
2) Shower needs to be cleaned.
3) Pipe in floor drain is to high.

4)' Clean and buff vinyl tile floor.
5) Shower head leaks.
ENTRY H~LL
1) Add hole in threshold for locking bolt on west storefront
door.
2) Rust, paint, and/or tarnish on front door hardware: closer,
pulls, hinges. Peel coating off of kickplates.
3) Door closer cover is cracked.
4) Provide keys to storefront doors.
5) West door stop on storefront doors is loose.
6) Set screw is missing from top hinge of active front door.
7) Need additional weather-stripping to seal air leak through
front doors.
8) Adjust front doors and deadbolt.
SWITCH,EAR ROOM
1) Adjust east side exterior doors - won't close, Set weather-
stripping closer at sides and top.
2) Door knob to entry hall is dented - replace knob.
3) Tile is loose in front of Pump #3 starter cabinet.
4) Caulk all tile cuts.
5) Clean and buff vinyl tile floor.
6) Seal crack at wall/ceiling joint.
PUMP ROOM
1) Trash and wasp nest in louvers needs to be removed.
2) Grate covering Pump #4 floor opening does not lay flat.
3) Heater on east wall has damaged heater coils.
4) Caulk opening on suction lines between pipe and shroud.
5) Patch broken concrete on pipe supports.
6) East personnel door needs adjusting.

7) Clean rust and paint from door hardware - all doors.
8) Add rope for manual operation of overhead door.
9) Copper piping on Cone valves is leaking - replace with brass
per pending field change.
10) Some of the fluorescent lights are burned out and need the
bulbs replaced.
11) RTD's need to be connected on pump bearings.
12) Valve Closed limit switch on Cone Valve #2 is broken.
ATTIC
1) Remove plywood which is obstructing flow through attic
exhaust fan #3 (east side).
2) Clean and remove trash from the pump room exhaust fan
enclosures.
3) Remove trash - coke cups, lumber scraps, etc. from attic and
rafters.
EXTERIOR STOREROOM
1) Install cover on phone cabinet.
2) Light does not work. (Please submit an estimate for field
change to add a separate light switch for this light).
EXTERIOR OF BUILDING
1) Extend all downspouts to within 4" of splashblocks.
2) Light fixture on front door (east side) is crooked.
3) Resurface center soffit at front door and rework the edge at
the backside of cast stone signage.
4) Touch up discolored areas of cast stone.
5) Hole in mortar on west side between stepped concrete grade
beam and brick.
6) Repair gutter joint on west side.
7) Clean and repaint downspout (middle) on west side.
8) Replace insulation on suction lines and provide water seal.
9) Provide weep holes over all lintels, 24" O.C.

.10) Electrical outlets under suction pipes need to be raised
above grade, weatherproof gaskets and GFI installed.
11) Electrical vault between suction line #2 and #3 needs to be
raised to grade.
12) Insulation covering on suction #2 is bent at ground level.
13) Clean and repaint southeast downspout at top. Reset splash
block.
14) Recessed light fixture (east of front door) is loose.
15) Caulk bottom of front windows.
16) Install weep holes over louvers - 24" O.C.
17) Several shingles appear to be missing on the roof at various
locations.
18) Heat trace and thermostats need to be connected on suction
lines.
19) Roof and ridge line appearance and aesthetics not in
compliance with the contract documents. Pending Change Order
to resolve this matter.
GENERAL BUILDING NOTES
1) Building needs to be thoroughly cleaned prior to City
assuming responsibility for internal cleanliness. This
includes dusting all equipment (including inside of
cabinets), cleaning all glass windows, all sinks, toilet,
shower, mini blinds, and doors and frames. Vinyl tile
floors should be cleaned and buffed. Pump room floors
should be mopped and all debris removed from discharge pipe
vaults.
2) Touch up paint on walls, doors, and door frames as needed.
Thoroughly clean and remove paint and other materials from
hardware (hinges, knobs, deadbolts, thresholds, etc.)
especially the door at the southwest corner of the pump
room.
SITE WORK
1) Sweep and clean drives.
2) Raise depressed area west of flow meter vault and adjust
valve pedestal to correct grade.
3) Install drain line on de-humidifier in flow meter vault.
4) Provide key to bilco door lock on flow meter vault and surge
valve vault.

5) Adjust grades to remove depressed area in front of
decorative fence - northwest corner.
6) Adjust grades to level of sewer cleanout at northwest corner.
7) The brass monument set in concrete for a property corner
(northwest corner) cannot be located - if this has been
disturbed or removed, monument must be replaced and reset.
8) Electrical vault (northwest corner) is full of water.
9) Concrete valve pedestal on 16" valve (west side discharge)
is broken.
10) Surge Valve Vault needs to be cleaned. Also, the lid to the
vault needs to be cleaned out and the drain fixed.
11) Wall plug in surge valve vault needs a cover.
12) The second street light from the north remains on during the
day when on photocell operation.
13) Heat Trace tape and duct seal needed in DWU Pressure Point
cabinet.
14) Investigate and repair possible leak on 36" influent line,
south and west of turnaround.
15) Regrade and smooth areas along and adjacent to concrete
drive, east and south of tank.
16) Topsoil contains excessive amounts of rock and other debris.
Section 02200 of the specifications details the replacement
of topsoil and objectionable materials. The front of the
building, west side, areas along the drainage ditch, areas
along the 36" line, and along all concrete drives need to be
addressed.
17) Remove construction debris from adjacent properties - east
and north of the pump station.
18) Adjust grades between tank valves and turn around area at
the northeast corner of the tank.
OFFSITE WORK
1) Remove concrete debris from along Sandy Lake Road.
2) Re-install drainage inlet at shopping center entrance.
3) Replace driveway to existing pump station and ROFC.
Temporary security fencing.

4) Backfill bore pits and complete clean up along offsite
waterline - To be done when water level subsides.
5) Pressure test 36" offsite waterline - To be done after
completion of #4 above.

Operation~
1) Instrumentation, telemetry systems, alarm systems (local and
remote), and automatic operation and sequencing of pumps.
2) Surge Valve operation.
3) Emergency shutdowns of running pumps.
4) Two pump operations.
5) Low level alarm, low level lockout, and by-pass on ground
storage tank.
In addition, we are attaching the punch list items from the Water
Utilities Department and Electrical Consultant. There are some
overlapping items on these list. Please proceed with the
remediation of these items within thirty days. In addition,
Section 01700; Paragraph 1.4 details the necessary close-out
submittals for this project. Please begin assembling these
documents to aid in the close-out of this project.

A. Pumps and Motors
1. Connect all pump bearing RTD's.
2. Winding RTD #3 in pump mtr #3 is not working. Reliance
thinks that their RTD is defective.
B. 480V Motor Control Center
1. Install grounding bushing on each conduit upcomer into
bottom of MCC and bond to ground bus. Refer to Section
16450 and N.E.C. Article 250
2. IQ Data Plus unit on active 480V feeder does not show a
reading for MVA for I = 50 Amps.
3. Seal and equip conduit risers as specified under Addendum #2.
C. Switchgear
1. Install grounding bushing on each conduit upcomer into
bottom of swgr. and bond to ground bus. Refer to Section
16450. Seal risers per Addendum #2.
2. Finish labeling of control wiring.
3. Label each door mounted device to identify its function.
Refer to Section 16360.
D. Pump Room
1. Replace and/or repair lighting fixtures with burned-out
lamps.
2. Replace ventilation thermostats. Specified T stats that
were specified to be adjustable were not supplied.
E. Wiring
1. Replace wiring in pole lights with indicated wiring per
Detail 3, Page 32. Ground EGC and pole as shown.
F. Instrumentation
1. Install grounding bushing on each conduit into
instrumentation console and bond to cabinet ground.
2. Seal ends of conduit as specified.
3. Fix momentary relay dip associated with faulty motor call-
for start of high service pump motors.
4. On-site DWU Pressure Meter Vault at GSR.
A. Install specified heat tracer on small water piping as
indicated.
B. Seal conduits and to prevent vermin ingress.
C. Caulk around base.
5. Program AB alternator to correctly alternate the H.S. pumps.
6. Correct on/off level control points for H.S. pumps. They
turn on and off at wrong levels.

Dear Mr. Ratliff:
Attached please find two (2) copies of the proposed dedication
plaque for the Village Parkway Pump Station. The elected
officials listed are those who were in office at the time the
contract was awarded by the city Council (December 1988). Please
review the proposed plaque and make any changes which you feel
are necessary. If you desire, other City officials may be added
and/or the elected officials changed to represent either the
current council or the council which authorized the bond program.
Please return one copy with any corrections to our office at your

Village Parkway Pump Station - Change Order No. 5
Dear Mr. Goram:
Attached please find six (6) copies of Change Order No. i for the
Village Parkway Reservoir and six (6) copies of Change Order No.
5 for the Village Parkway Pump Station. Change Order No. i for
the Reservoir adds 345 LF of chain link fencing at a cost of
$5,406.15 ($15.67/LF) to the tank contract. Change Order No. 5
for the Pump Station deletes 345 LF of chain link fencing at a
savings of $5,406.15 ($15.67/LF) from the station contract.
These change orders represent a net of'no additional cost to the
City of Coppell for the combined projects. Combining all chain
link fencing into one contract will provide a uniform appearance
for the site security fencing. If left in two contracts, the
sections may not match in color or appearance of the
appurtenances.
Please review these proposed change orders and if satisfactory,
forward them to Mr. Ratliff for his approval. After execution by
the City, please return all copies to our office for further
processing. If you need additional information or have any
questions, please feel free to contact us.
