ITEM 1: Call to Order - The meeting was called to order by Larry
Davis at 1:49 p.m..

ITEM 4: Larry Davis made a public apology for failing to have the
utility companies (i.e. Lone Star, Paragon, GTE) contacted. (Staff
shortage- unavoidable).
ITEM 5: TU Electric - Glenn Fabian with TU Electric made the comment
that all plans were okay. The only exception is that TU is in need
of documented information stating that the plans include the design
be able to expand.
ITEM 6: Contractors - Discussion concerning the testing of the
utilities. It was decided that the video test requirement would
be waived due to the fact that only 30' of sewer was being laid.
The street cut and street patching was then discussed and Larry Davis
referred the contractor to Per Birdsall, Street Superintendent for
permitting.
Contractor asked to use 6" x 2"- 3 way iron fitting for the 2" tie
in into the 6" line, Water Utilities Superintendent, Howard Pafford,
asked that he use a dull 6" x 2" compact ductile iron fitting (4
way)- all parties in agreement. The Superintendent also asked the
contractor that the fire hydrants match the existing hydrants in
the area. Contractor will submit a drawing from Dalworth (6'd -
5'w and 1' from outside of wheel on valves). Howard Pafford informed
the Contractor that he could get 2" water service details from his
office.
ITEM 8: Paving/Excavation - Larry Davis informed contractor densities
are required. Xerox copies of the test will need to be distributed
to the Inspector, Engineer and to Murray Construction.
Mix design required for concrete, submittal from producer needed.
Sanitary sewer lines will be PSI 4 lbs.
Infiltration- Mandrel / 33 ft. Inspector to be present.
Televising- Waived

Public Protection: Barricades (traffic/work zone) needed.
Flagmen will be required on site - no exceptions-.

ITEM 9: Warning - Initial Notice of Problem will be verbal and written
notification, second notice of problem will be asked to halt all
work/ actual stoppage.
ITEM 10: Fire Dept. - Siamese connection was discussed. Check with
Dale Jackson- 240 ft. to the fire line (i.e. St. Ann's Catholic
Church, Minyards, Stock Tab)

This letter will serve to summarize the variances approved by the City
Council regarding the above at the council meeting of August 8th:
1. Concrete paving of parking lot and fire lane approved to be five
inches thick
2. Five inches of crushed rock as a substitute for five inches of
concrete paving in parking area bounded by fire lane (see
yellow highlight on attached site plan). Concrete paving in this
area will be scheduled for Phase II consmaction.
3. 410 linear feet of sidewalk along Heartz Road to be omitted
until Phase H consuuction (see attached highlighted area).
This should assist in ke~ping'sll concerned adequately informed. If
