Summary of Services Performed (field test data, locations, elevations & depths are estimates) & Individuals Contacted.
The undersigned arrived on site, as requested, to observe the placement of 18 cubic yards of
3600 psi concrete for Vanbebber Drive Appeoach. Please see attached sketch.
The required test limits were determined to be a slump of 3 -5 inches, air content of 3 -6 %, batch
to placement time of 90 minutes, and concrete temperature range of 50 -95 degrees Fahrenheit.
Concrete observed on this date appeared to be placed in compliance with project specifications
with regard to temperature, air content, slump and batch to placement time.
One set of laboratory cylinders were cast for compressive strength testing in our laboratory.
Prior to the placement of concrete, reinforcing steel appeared to be placed in accordance with
the project plans and specifications.
