The following items were found to be deficient:
1. Random crack - Two (2) at West side of drive approach and two (2) at East
side of parking lot
Back of curb busting out at saw joints
Four (4) locations - three (3) on East side of parking and one (1) on West side
of entrance island
Expansion joint does not pass through curb - southwest edge of northern most
island
4. Curb damaged on next to last center island
5. End of curb damaged at concessions pad
6. Apply curing compound to all previously patched curbs to bleach concrete.
7. Damage to curb - northeast corner of parking lot.

Damage to paving east of entry drive.
All irrigation sleeves need to be exposed and marked in their proper location
with a brass pin.

In compliance with the contract, rye grass was installed but did not receive a
uniform stand. All areas with the exception of those north of the concessions
pad require reseeding with Bermuda Grass per conversation on March 26,
1993. Based upon my conversations with Texas Environmental Managment,
it will be necessary to temporarily irrigate with above ground lines to establish
the Bermuda.
11. Verify grades of concession building pad. Appears to be low on the west side

The soil grades at the northwest corner of drainage flume need to be reworked
for ease of maintenance.

All left over concrete, rock and debris shall be removed from the job site.
Hay can be broken apart and spread over area.
14. Provide keys for the electrical boxes.
15. Two of the three 2" meter sets need an 18" meter can placed over lower cut off
on pressure side of setter.
16. 8" water main line valve needs pad repoured and brought to grade.

18. The two cleanouts on the sanitar3~ sewer mainlines need concrete removed
from caps and bolts. Also, if the cleanouts are only 4", was this called for in
the construction plans?
19. Pavex appears to be settling at edges, but no repair is necessary at this time.
20. Repaint all parking and firelane striping if peeling or paint is thin.
In addition to the above, please, provide an uncondit'ional waiver of liens or affidavit
of payment for Gregory Construction.
Please be aware that final payment and acceptance of this project is subject to
completion of all punch list items and formal approval from the Coppell City Council.
A recommendation for accepiance from our office and Public Works will be submitted
to Council at the completion and approval of the above punch list:
