I represent Capitol Indemnity Corporation ("CIC"), the bonding company
for Advantage Contract' Services, Inc. ("Advantage"). We need information from
you concerning the status of the project referenced above. Attached is a
questionnaire that will assist Capitol Indemnity in assessing the status of each
project. Please. complete, as soon as possible, and fax to my office at (713) 650-
2400.
It is also important that you notify my office of any payments made or
payments that you are making to Advantage in order that I may track the status
of the job. Please copy my office on any pay applications and/or evidence of
payment from you to Advantage. This is very important.
