I. Paragon Cable has a pole in th~ way of thc box culvert which is hindering box construction
progress. A meeting between the City and Paragon is needed.
It has been decided to move GTE facilities rather than alter thc storm drain design at thc box
h~tcrnl.
3. GTE h~ not lowered conduit at stn 5+54. All other inlet conflicls have been adjusted Also. there
is a new GTE pedestal that may nccd t0 bo adjusted t0 new grade.
4. Thc waterline is installed m~d is fully functional.
5. The City ]las advised EBCC to keep access open to all fire hydrants during constn~clion illld I.~
keep these nnd other existing services fully functional at all times.
6. The City atlvises EBCC to maintain inlets and erosion control during conslruction.
7. *Humphrey & Morgan will construct thc box. Thc City needs wrillcn notification of the
subcontractor substitution and a summary of qualifications.

8, Erosion control is a major concern at the downstream end of thc new box. No silt or debris should
be allowed to travel pasl the culvert under Westminstcr Way at any time during construction.
9. Letters o[ request will be needed by the City by the Wednesday pri{~r Io any weekend work.
I 0. *Thc cement-stabilization d~ application is a concern tbr subgradc treatment, duc to thc dust that
would bc created during placement. EBCC d~s not cu~ently have the capability Ibr wet CSB
application. Application of the treatment at night is a cm~cem due to the high noise thcmr. In order
to minimize impact on the surrounding public, the City would like to explore thc possibility o!'
chzmging the pavement section m a 5- I/2 sack, 3,5~ psi mix design, in lieu of subgradc treatment.
Costs I~r diflbrenl options will be provided by EBCC.
I I. Lime treatment shall be used fi'om Sandy Lake Road and end at box culvert.
12. Public interfitce has been successful. To this point, there have been no citizen complaints.
~3. *'Fhe City requests that EBCC add a "one way" sign on 121.
I. 4. *Thc City requests that EBCC correct the erroneous disttmces on the box detour signs ~md lo add
a 20 mph sign ahead on the fmxhest distance sigm from the detour ~o provide motorists with mort:

15. *The City needs submittal of a revised critical path schedule.
16. Good Friday will not be an official holiday for the City.
[ 7. Thc City has concerns a~ut thc drainage pi~ ~nea~ the new detour road and questioned EBCC
about drainage capacity. EBCC has expressed their confidence in the pi~, which is a 16" DIP.
