MEMORANDUM

RE: W.T. Cosby Library Walk Through
Staff performed a walk through at the above referenced location
and discovered the following deficiencies;
Approach needs sealed
Crack in concrete on south side of radius in entrance
" in concrete first island
Gouge in curb south of entrance
Crack in paving and curb at first island on north side
Crack on east end of island
Steel sticking out of curb behind dumpster
Video/Cable conduit pipe full of dirt
Sealer on expansion joint coming out
Parking lot needs to be sealed
Please make sure the contractor receives a copy of the above
listed deficiencies. Contact our division when a follow up is
needed. Thank you.
