Description of Operations:
On this date, representatives of GEE Consultants, Inc. visited the above referenced site to perform a final inspection of the single
ply and standing seam roof systems as well as building interior to check for leaks. The purpose of our observations were to determine
conformance with plans and specifications and whether corrections of outstanding items noted in our Janua~, 31,1995 inspection had
been made. Outstanding items are as follows:
1. Joint cover plates at perimeter coping of single ply system still missing. This may be an approval detail.
2. Few areas of poor securement of top clip at standing seams throughout main roof. Top clips should lock over standing seam
panel cleats. Numerous locations between 7th and 8th panel west of northeast dormer.
3. Need to clean EPDM adhesive from base of west side of north skylight roof.
4. Notches still exist in panel standing seams at radius change (reverse curvature) areas above dormers.
In addition, observ'ations of the inside of the building were made to look for signs of potential roof leaks. Stained ceiling tiles were
observed at the following locations:
1. Southeast corner of building.
2. East end adjacent to fire sprin~er head.
3. Small closet at northeast corner of building.

Access to rooms along the east side of the building entry, hall could not be gained due to construction activities.
The above observations were verbally brought to the attention of Mr. Terry Bane with Diversified Construction. Mr. Bane stated
the stained ceiling tiles occurred from an overnight rain. He stated he knew of no other evidence of roof leaks. We recommended
he contact his roofers to repair leaks occurring at the stained ceiling tile locations.
