On this date, September 21. 1994, a representative of GEE Consultants, Inc. was present at the above referenced
project to perform inspection at the following locations:
1. North lane of parking lot on north side of building, west half.
The following discrepancies were noted and called to the attention of Mike Sherpinskas. a representative of
Diversified Construction.
Slip dowels not used for construction joint.
The above discrepancies were corrected as noted.
