Results
On this date, May 19, 1994, a representative of GEE Consultants, Inc. was present at the
above referenced site to visually inspect the exterior grade beams from grids 2.6 to B.8 at
the above referenced site in accordance with plans and specifications.
The following discrepancies were noted and brought to the attention of IvIike Sherpinskas, a
representative of Diversified Construction Services:
1. Wrong size top and bottom rebar on GB16
2. Mud on curved rebar of GB12
3. Top hook bars not extended to within 2" of pier face
The above discrepancies were corrected as noted.
