On this date. May 29, 1994, a representative of GEE Consultants. Inc. was present at the
above referenced site to visually inspect the Grade Beams at the tbllowing locaitons:
Grade Beams 14, 4, 18, 10. 9, 17. 6, 3 and 7 between grids
C-12 to J.4-7.8
The tbllowing discrepancies were noted and brought to the attention of Charlie, a
representative of J.L. Burney Construction Company.
i. Improper spacing of stirrups
2. Missing #9 bar in bottom of GB-19
3. Dirty pier tops
The above discrepancies were corrected as noted.
