On this date, May 24, 1994, a representative of GEE Consultants, Inc. was present at the
above referenced site to visually inspect grade beams at the following locations:
Grade Beams 3, 6, 17, 15, 7, 8, 5, 1, 9, 10, 21, 11, 12, 13, 14, and 16 between grids
J.4-7.8 to A.5-2.5
The following discrepancies were noted and brought to the attention of Charlie, a
representative of J.L. Burney Construction Company.
i. Improper spacing of stirrups
2. Dirty pier tops
The above discrepancies were corrected as noted.
