Date: September 10, 1997 Subject: Village Parkway Pump Station Additions Status Meeting A meeting was held on Tuesday, September 3, 1997 to discuss the status of the Village Parkway Pump Station Additions. The agenda included: 1. Unresolved punch list items 2. Discussion of Liquidated Damages 3. Repairs to the Deforest Lift Station 4. Closing of the construction contract In attendance were: Mr. Ken Griffin, P.E., City of Coppell Mr. Mike Daige, North Texas Municipal Construction Co. Mr. Gary Hendricks, P.E., Shimek, Jacobs & Finklea, L.L.P. Discussion Item 1: Unresoived Punch List Items: (refer to memo from GCH to Ken Griffin, dated 8/;28/97) The punch list(s) were discussed in detail at this meeting. The tabulation below summarizes the action expected to be taken on each item (referred to by number). 1. Items to be Completed by NTMCC prior to final acceptance of the project: Original Punch List Items: 1,3,4,5,6 New Items: 4,8,9, 13,15,16 2. Items to be completed by NTMCC under the Warranty phase of the project: New Items: 1,2,3,5,6,10,11,17 3. Date: September 12, 1997 Subject: Village Parkway Pump Station Additions Status Meeting Item 2: Liquidated Damages: It was generally agreed that the date of substantial completion for this project was July 28, 1997, the date in which the final correction was made to the IQ 1000 was made. Both the City and NTMCC agreed that a change order would be executed which reduces the final contract amount by $2,275, eliminates pay items 4, 5 & 6 and extendes the contact time by 110 calendar days, if NTMCC can satisfactorily complete the remaining items of work as discussed in Item 1 above. The project is to be acceptable to the City prior to Sept. 30, 1997. Item 3: Deforest Lift Station Repairs: The City will submitt a interim cost report to NTMCC so that it can be forwarded to their insurance carrier. From: Gary Hendricks ~ Date: September 12, 1997 Subject: Village Parkway Pump Station Walk-through on September 10, 1997 A walk through of the Village Parkway Pump Station Additions was conducted on September 10, 1997. In attendance were: Mr. Matt Cast, City of Coppell Mr. Larry Davis, City of Coppell Mr. Mike Daigre, North Texas Municipal Construction Co. Mr. Bill Bigham, North Texas Municipal Construction Co. Mr. Gary Hendricks, S.J.& F. With the exception of the those items listed below, the punch list items published in the S.J. & F. memo to Ken Griffin, dated August 28, 1997, have been completed to the City's satisfaction: 1. Handrail at the motor control center: Although NTMCC attached a handrail to the wall as requested, it does not function well for the intended purpose. NTMCC agreed to move the ladder over approximately 6-8 inches in order to center the ladder between the left and right handrails. Matt Cast and Larry Davis agreed to this solution. 2. The emergency kill switch in the pump control cabinets for pumps 5 and 6 still is not working properly. It will de-energize the pumps while in the hand mode only. It did not function in the "auto" mode. NTMCC agreed to correct the' problem. 3. There is still about 3/8" of standing water in the middle of the new driveway turn-out. Bill Bigham and Larry Davis agreed that extending the concrete grinding work further up the drive would solve the problem. NTMCC agreed to complete this work. We also reviewed the SCADA system status and found a few problems. That work will be identified in a punch list to be published at a later date.
