MINUTES OF PRECONSTRUCTION MEETING

Items discussed are as follows:
~1. Contractor filled-out and submitted Bid Affidavit.

Contractor does not have base limit of $600,000.00 car insurance, has base of
$500,000.00 but the Contractor does have a five (5) million dollar umbrella which was
approved.
4. Notice to Proceed was issued in wriiing by the City of Coppell on 17 October I997.
~,~'-~ '~.~ Contracts need to be signed and 100% Payment and Performance Bonds need to be
submitted.
6. Dan Whitficld is to be the Project Manager for Moisture Shield. Jeff Miller will be the
Superintendent for Single Ply Roofing. David Doyle or Al Sistrunk will be
Superintendent for remainder of the projects.

7. Verbal approval for Change Orders will be authorized in order to expedite the project, but
a written Change Order must be approved and on file as soon as possible.
8. A list of major suppliers and sub-contractors is to be submitted by the Contractor.
9. A construction schedule is to be submitted by the Contractor.
10. Sunday, December 6~ is thc City of Coppell's Christmas Parade, it is hoped that the
Contractor will bc offthe front of building that day.
11. Christmas lights are to be removed by the Contractor but only perimeter lights will be
replaced. The vertical strings of Christmas lights will not bc rc-installed.
12. Thc East Parking will be barricaded by the City of Coppell for usc by the Contractor for
storage, etc. The last row of parking in the front aI the street can be used by crew
members if needed.
--~ 13. Th~ City of Goppell is to flag sprinkler heads and supply a site plan of the sprinkler
system for the Contractor.
14. Any damage to the sprinkler system or landscaping is the responsibility of the Contractor.
A run/test of the sprinkler system will be conducted by the City of CoppelI at the
completion of the project.
15. Thc Contractor plans to re-roof the steep shingled area first.
16. The Contractor plans to use bituthane membrane as a temporary flashing to protect
against leaks.
17. Thc color chosen for shingles is Slate Blend. The color choice will be verified by Ms.
Shed MoinO by October 2g~', 1997.
18. Vents and stacks on single area will be painled to blend into roof.
19. Valley metal to matgh shingles and him and will be Kynar 500 Pre-painted.
20. Contractor will take photos of any parking lot and entrance cracks or other problems prior
to deliveries and the start of the job.
I*'~.g~,,o.,~21. The City of Coppell is responsible for movement and replacement of satellites. The
Contractor is to give at least one (1) days notice.
0.z~,~4. ,.~=~._~ The large radio antenna and associated wiring is to be removed by the City of Coppell
¢~.,i before application of clastomeric coating on concrete equipment pad.
23. Thc Contractor can access elcctrical service at the job site but the 220 high amperage
required for the single ply roof seaming machine must be supplied by a Contractor
supplied gcnerator.

~ ~ Contractors persom~el.
25. The City of Coppell's hours are 8 AM to 5 PM, Monday through Friday. However, thc
Contractor will be issued a back door key. No code or security procedures are required.
26. The Contractor is to u~e counter weight system 1bt safety lines as there is no structural
davits to attach to.
.-~27. The City of Coppell is to make available the as built structural plans so the Contractor
can check locations of I-bearns and joists for staging materials on the roof in cupola area.
28. Ali work to bc cleaned up daily.

29. Thc Contractor shall 10c responsible for leaks when thc job starts. Areas pointed out to bc
leaking at last rain were both small fiat roof areas at the rear of the building and the small
fiat area on the front northwest corner.
30. Post a sign at the Justice Center walk area to Town Center if working that area of
building.
31. Safety rules were covered and OSHA roles will apply.
32. A written safety program is to be submitted to PCI and the City of Coppell.
33. MSDS shee~ must be submiRed with submil~ls.
---~34. Submittals are not rea~dy yet but are promised to be delivered to PCI's office by October
29'~, I997.
35. Single-ply Manufacturer is Sarna.fil and they will be on the project periodically and will
also perform a final inspection.
36. The Contractor has a 24-hour telephone answering system for emergency and after-hour
purposes which will automatically contact the person on call. Leonard Downing will be
the emergency contact for the City of Coppell and can be reached by his pager number
(214) 897-1590.
~ The City of Coppell is to supply the electrician for dis-connects and re-connects of
electrical lines and service boxes, as required.
38. It was requested that the Contractor submit a price for Change Order to reset coping
stones and seal joints at the front entry planters, drill weep holes in planters and cut out
and seal cracks in plaza at the front entry.
39..~ It was requested by the Contractor that the hedges at the northeast and northwest sides of
~ --~' the building be trimmed back as they are too close to the walls. Also, it was pointed out
tv,, that the City needs to crop back the ground cover/Ivy as it is growing up the wall. The
City of Coppell stated that they would take care of same.
40. A complet~ interior and exterior job walk was performed.
41. The meeting was adjourned.
