D. DEFICIENCIES AND CORRECTIONS:
1. STATION NO. 3
A. PSA has reviewed the scabbed 2x'~ at the truss tails and has no further objections to the soffit framing. PSA
feels that the fascia is now adequately attached back to the structure.
B. PSA informed Boyd that a good portion of the fascia board would have to be replaced because of its poor
quality. PSA also informed Boyd that the Owner wants to change the fascia from pine to cedar.
ACTION: PSA to issue RFP.

C. Concrete grade beam at O.H.D. was not poured 1/2" below f'mished floor and the steel angle which forms a
waterstop was set on the outside face of the grade beam. This results in a flush condition from the drive
through to the finished floor of the Apparatus Bay, Boyd presented a waterstop solution which is comprised of
a 1/2" x 4" galvanized steel plate sealed and screwed to the floor.
ACTION: The C.F.D. has decided not to add the steel waterstop this time. When the doors are
installed and the building is close in, PSA will monitor the need for a waterstop.
D. Concrete drive at both Apparatus Bay approaches were poured onto the brick ledges between the overhead
doors, and the pipe bollards were not set prior to the concrete pour of the drives.
ACTION: Boyd to replace a 5' - 0" wide strip of concrete approach on both sides of the App~u'atus
Bay in order to remove the concrete from the brick ledge, remove the steel waterstop
from the outside of the grade beam, and install the pipe bollards.
E. PSA and the C.F.D. questioned whether the proper positive slope toward the trench drains is achieved at the
Apparatus Bay floor.
ACTION: Boyd to provide spot elevations of floor for PSA to review.
F. PSA informed Boyd that masonry openings for the air louver in Gear Storage 119, combustion air louvers in
Janitor 120, the dryer vent for Decon 116, and through wall A.C. unit in Shop 117 were not constructed.
G. Concrete drive was poured over the brick ledge corner adjacent to Dayroom 107.
ACTION: Boyd to modify per ASI 05 dated 1/9/97.
H. Concrete drive adjacent to visitor parking has a crack the full width of drive.
ACTION: Concrete drive section will have to be replaced.
I. The existing water valves at the approach on Parkway Blvd. has been covered with concrete.
ACTION: Existing valves will have to be raised flush with the top of drive.
J. PSA informed Boyd that, in most cases, the rim joist along the fascia is screwed into the sheathing only: Rim
joist is required to be screwed into metal studs.
K. Sheathing is required above the window openings along header at perimeter walls.
2. STATION NO. 3
A. Concrete grade beam at O.H.D. was not poured 1/2" below finished floor and the steel angle which forms a
waterstop was set on the outside face of the grade beam. The concrete drive approach on both sides of the
Apparatus Bay was poured on top of the brick ledge. _

ACTION: Boyd to replace a 5' - 0" wide strip of concrete approach on both sides of the Appm'atus Bay
in order to remove the concrete from the brick ledge, remove the steel waterstop from the outside of the
grade beam, and install the pipe bollards. Boyd to set a new steel waterstop on the inside face of the
grade beam at elevation 100' - 01/2" and taper the concrete down to elevation 100' - 0".
B. PSA noted that the//4 grade beam steel that ties into the slab was set too high at the three eastern most
overhead doors on the south face of the Apparatus Bay. As a result the top of the inside edge of the grade
beam has been chipped off in various locations. The bottom row of//4 steel is missing.
ACTION: Project Structural Engineer to review and make recommendations.

C. PSA noted the two areas of the concrete drive collect water. These are the transition areas between the drive
and the first four parking areas just off Condor Drive.
ACTION: These areas will have to be worked with a grinder to achieve a positive water flow.
D. PSA noted that the fire hydrant was installed facing the building.
ACTION: Hydrant to be rotated 180 degrees.
E. Existing water valve covers will have to be lowered at Condor approach from MacArthur. Top of value cover
to be set flush with surface of Condor Drive.
E. ITEMS DISCUSSED:
1. PSA informed Boyd that no responses to RFP's #5 and #6 have been received.
2. Job sign at Station No. 3 still reads Station No. 2.
ACTION: Boyd to modify sign.
3. PSA informed Boyd that the UV rays from the sun will damage the ice and water shield placed on the roof at Station
No. 3.
ACTION: Boyd to consult manufacturer. If the material has been exposed longer than recommended, the
shield will have to be replaced.
4. PSA informed Boyd that a Change Order for additional pier casings will be processed.
5. PSA requested that Boyd uncover the core holes drilled through the grade beams so that the application of the concrete
sealer can be verified.
6. Boyd informed PSA that a concrete pour for Station No. 2 was scheduled for January 27, 1997. PSA stated that the
structural engineer will have to review the project prior to any concrete pour.
ACTION: PSA to schedule a review by the structural engineer.
7. PSA requested that the concrete drive leave out at Station No. 3 be poured as quickly as possible or secured with an
adequate barrier.
8. PSA requested that Boyd remove construction debris along Parkway Blvd. and keep the gutter free and clean.
