General:
A. This-RFP is issued to define the scope of the proposed modification in Drawings and
Specifications for a contemplated Change Order to the subject Contract. The work required by this
modification, when duly authorized, shall be executed in accordance with the Genea'al
Requirements of the original Contract Documents.
B. No work shall be started until receipt of an authorization in the form of a Change Order.
C. Please prepare and submit for review a Proposal for change in the Contract Amount and time for
the modification to work herein described in accordance with the Contract Documents of _this
Contract.
Description of Proposed Modification:
1. Provide 6' concrete encasement at 4' plumbing line under concrete drive at Fire Station No. 3
(Mac site).
