Re: Village Parkway Pump Station Addition
Final Inspection
Dear Mr. Daigre:
On May 27, 1997, a final inspection of the Village Parkway Pump Station was conducted and a list of items
that needed to be completed or corrected was generated and documented in a letter dated May 27, 1997.
During our weekly visit to the Village Parkway Pump Station, a review and inspection of these items was
conducted. The following items remain incomplete:
1. A screen or guard is required on the ventilation fan (EF-4).
2. Pump No. 5 -- power conduit casing is ripped, burned or melted and should be replaced or repaired.
3. Fine grade and re-seed areas disturbed by construction, particularly the area behind (west of') the
disinfection screening wall, between the existing ground storage tank and access drive, and along the
trench line for the DWU sample and solution lines.
4. The trench for the under ground electrical conduit which was re-routed to the east side of the building in
this centTact, has settled near the existing electrical manhole.
5. Add the required flush valve to the strainer in the cone valve control cabinet (See Matt Cast or Ted Dull)
6. Refer to North Texas Municipal Construction Co., shop drawing submittal No. 5050-1 -- Miscellaneous
Steel, dated 6/04/96. The City has requested that handrails be added to the MCC platform on the north
wall to assist in elirobing the ladder reloeated there by change order. NTMCC indicated they do not have
adequate railing to accomplish this request. Drawings E1 and E2 in this submittal shows a 90" long
aluminum handrail to be connected to the north pump station wall with three connectors. It also shows 42
aluminum post and three 60" long handrails along the proposed stairway. None of this railing was
constructed when the stairs were eliminated and the ladder reloeated, It seems from these aluminum
pieces and fittings not used that adequate handrail material is available to complete the handrail at the
new ladder location. Also, this submittal shows a safety chain across the ladder opening at the south side
of the elevated platform. We did not notice this chain during our final inspection.
7. Repair the cracked curb near the driveway tuna-out.

8. As noted in our letter dated June 11, 1997, the finished grade at the end of the driveway turn-out is to high
and a section of the driveway approximately 25 feet in length is fiat. We recommend NTMCC remove
the flat section by grinding along the flat section to a depth of no more than 0. 10-inches at the end of the
drivewway turn-out in order to achieve positive drainage. We are enclosing a copy of tile June 11, 1997,
letter and its attached exhibit for your use.

Dear Mr. Davis
As you re~.uested, we have completed the As Built survey of the driveway turn-out constructed by NTMCC
with the Village Parkway Pump Station Additions project. This survey was conducted to verify the finished
grade elevations of the constructed driveway. As shown on the enclosed exhibit, the finished grade at the end
of the drive way is high (Survey Point 20) and a section of the driveway approximately :25 feet in length is fiat
(Survey Point 14 to Survey Point ~21). In order to correct the problem, we recommend NTMCC grind or
remove a section of the driveway from survey point 14 to survey point 20 until the finished grade is in
conformanc~ with the proj~t plans or positive drainage is achieved.
