Central Expwy., Suite 260 Dallas, TX 75243 Re: Coppell Senior Citizen's Center PSA Project No. 4087.10 Dear Mr. Bailey: We are deeply concerned with Bailey Construction Inc:s (BCD inability to complete this project. This letter is written as a follow up to a project review meeting held at the Coppell Senior Citizens Center on Monday, January 6, 1997 between PSA, Bailey Construction, and the City of Coppell. To summarize what was discussed concerning the unfinished punch list items, we provide Punchlist No. 2 which is attached to this letter. In addition to the punchlist, we require the following items before substantial completion and the final application for payment can be released. As previously stated, please review the specifications for all final requirements to close the project. The following must be available on or before the Final review meeting. 1. Tile Flooring: The Multi-purpose room flooring is the largest outstanding issue for completion of the Coppell Sr. Citizen project. Change Order//2 was approved on December 11, 1996. This change order amended the contract by the following: 1. Prepare floor for replacement VCT tiles. 2. Remove and replace all damaged VCT flooring and associated wall base in an approximate 10 foot diameter with center point between two walls in Room 109- Multi-purpose Room Since December 11, 1996, no evident progress has been made on the flooring. Substantial completion can not be established with such a large area still incomplete. Bailey Construction has not'kept PSA informed on the progress of the flooring schedule. 2. (2) Complete sets of all required Owner Manuals and Warranties. Please review specifications for format and requirements. 3. One full set of as-built proj_ect documents including drawings and specifications. Please make every effort for a clean set. 4. Submit signed Consent Assurity from bonding company per specifications. 5. Submit Retainage Affidavit from contractor per specifications, . ~...:,~., :--:::-..' 7. After all specified items have been submitted, cdtiiOletEd, antl'.sx/bstantiat completion awarded, PSA will release the final pay application. We were extremely disappointed to learn that only half of the punch list items were completed by January 6, 1997, the deadline established by Bailey Construction and PSA. As stated in your contract with the owner, the date for substantial completion was amended to November 1,1996 due to the delayed flooring installation from water leakage. It is now 68 days beyond that date and very little progress has been accomplished. Let us bring to your attention the fact that liquidated damages can be assessed for $34,000 for your failure to meet the terms of your contract, as of this date. However, PSA will recommend to the City of Coppell to establish January 14, 1997 as the neTM date for substantial completion, and if Bailey Construction completes all outstanding items by this time we will recommend-that the liquidated damages be waived. If not completed, the City of Coppell will-collect all of the liquidated damages allowable under the contract and notify your bonding company' for failure to comply. We are extremely concerned about Bailey Constmction's lack of urgency concerning the completion of. this project. This project requires your personal attention and commitment to ensure a successful completion. (Please notify PSA of your intended schedule.). Fill broken CMU. Room 101 - Back Door Area . 14. Clean stains in carpet. Room 102 - Office a. Existing lock to be adjusted. 19. Secure plates. 20. Touch-up door paint by re-painting entire door. 21. Adjust door to open/close without scraping floor. Room 103 - b. Replace broken phone plate. c. Cover exposed wire. Room 104- 29. Rem_'ove hook, chain, 2 x 4 - paint and patch ceiling. Room 105 - Men d. Fix leak at toilet supply line. 31. Repair hole in door by lock. Room 106 - e. Seal around mop sink. f. Secure over pressure pipe to the wall. g. Seal around piping penetrating CMU wall. Room 107 - Kitchen 40. Touch-up paint on walls and ceilings. 41. Base missing at doorjamb by CMU. 42. Repeated comment 43. Remove return/supply grilles and clean. Re-install and fix screw 44. Clean wood trim edge on cabinets. 46. Patch/paint:hole in wall. Room 110 & Vestibule h. Re-apply sealant around mirror. Room 111 - Craft 54. Patch hole at old laundry area, paint area, & cut base around patch. 55. Cracked and scraped wall. 57. Repair door hardware on rear door so that door is operational without force. Room 108 - Electrical 1. Adjust tension on door. Room 109- i. Secure wood casework to each CMU wall. 65. Clean wood trim on millwork. 66. Flanges removed from wall and wall patched and painted. 70. Adhere VCT tiles throughout so surface is smooth. 73. Patch holes in CMU - north and west wall. 75. Touch-up paint - especially at wingwall. 76. Trim boards - adhere to CMU wall. 77. Clean storefront frame. 78. Clean grey paint off CMU. 79. Adjust door El07 to close correctly. 80. Paint conduit. GENERAL NOTES: 1. Tack down robber base throughout. 2. Touch-up paint throughout where indicated. 3. Secure and clean electrical plates where indicated. 4. Completed 5. Touch-up paint at all doors throughout where indicated or needed. 6. All touch-up paint to match base paint. 7. Senior Center: 1. Could not confirm T-stat for new unit was controlling unit. Verify wiring to unit. 2. Exit sign over exterior door (rm. Ill) interferes with traffic through door. Relocation to beside door is required. 3. One kitchen light does not switch. (This is possibly an existing night light.) The owner may want to be ~ble to switch it off with the others. 4. Fan controls still appear to be lighting din~ners rather than fan sp~ed controllers. 5. Item No. Il from previous report, lhe telephone board was not installed. The owner is working with the telephone co, They lore coordinating a residential service that will not require the board. Have board moved out of main room into Elec. Rm. until it is removed from site. 6. New Item. Need a new lamp in the light fixture in the corridor vestibule.
