Those in Attendance: Mr. Don Wertzberger Phillips Swager Associates (PSA) ROOF 1. Clean all miscellaneous debris, garbage, excess construction materials, pieces of scrap wire, screws, nails, bolts, scraps of flashing, etc. from all of the roof surface levels. 2. Submit copies of documentation to PSA from the modified bitumen roof manufacturer, Tarnko, and Berridge Manufacturer for their final Inspection Reports and Punch List items. 3. Executed roof warranties are still outstanding (SBS modified bituminous sheet roo£mg and standing seam room systems). 4. A formal report regarding previous hail damage as previously documented in PSA Memorandums, addressing the acceptability of the roof membranes (modified and standing seam) and any potential corrective action and status regarding the hail damage. 5. There is an area of ponding water adjacent to the mechanical unit (RTU 11) at the south end. 6. Additional touch up paint is required in several areas at the metal parapet coping. There are numerous areas in which there are scratches and gouges, as well as pealing paint. Completely sand, reprime and touch-up paint all areas of metal parapet coping. 7. Properly trim all excessive brick weep sash cords at the three lower roof levels above the canopies, at the south, east, and west entrances. 8. Additional sealant and caulking is required at all joints and splice pieces of the metal parapet coping. Sealant has been provided at several intersections and areas; however, at all of the splice pieces there is no sealant along either edge. 9. Additional securement and trim out metal cap flashing is required at intersections of standing seam roof with the top parapet pieces, as well as at all 90 degree corner flashing pieces throughout (approximately nine locations). 10. Submit certification of the lighting protection system. 11. Clean gutter troughs at the perimeter of the three lower level roofs above the entrance canopies of all debris, garbage, and scrap construction materials,etc. 1 12. There are several wood blocking pieces for horizontal pipe gas piping lines which do not have protective roof pads beneath them. In addition there are several blocks which are floating/suspended and do not come into contact with the roof. 13. The gutters at edges of all sloping standing seam roofs currently are holding water at several locations, up to 3/4", and are not properly configured or sloped for positive drainage to the downspouts. 14. There are several seams throughout the modified bitumen roof'mg - horizontal as well as vertical-along the flashing and cap sheet which appear to be loose and do not appear have a sufficient amount of bitumen. 15. Significant amount of rework is required at the comer transition and filler pieces atop of the parapet at right corners with the standing seam roof at the square boxes. 16. There is ponding water at two locations at the south end of the barrel vault at the flow- through scuppers - one along the east wall(D.5-1), one near the northwest wall (D. 14). 17. Additional sealant is required to close off the gap at the transition of the metal parapet cap with the vertical trim piece of the barrel vault along the south edge of both ends (D-1 and D4). 18. Roof membrane flashing appears very piece-meal and haphazard around the new roof exhaust fan installed directed above the Equipment Room 126. 19. There are four exposed nails in the brick veneer mortar joints up near the metal parapet along the east wall directly above the Equipment Room 126. Remove nails and fill with mortar or sealant. 20. The roof cap sheet protection pads beneath all wood blocking were to have been secured or hot mopped onto the roof membrane. As currently installed the protection pads are unsecured and are moving and being displaced, and will eventually be blown away. 21. Additional touch up of vertical membrane flashing and sealant is required around the perimeter of all roof top units and exhaust fan boxes. 22. Several of the pitch pans need additional topoff and float out of sealant to prevent water from eventually ponding within the pitch pan. 23. Walkway pads protection or pads were to have been installed in paths to all of the units as depicted on Sheet A300. 24. There are numerous visible dimples in the standing seam metal roofs throughout from the hail damaged previously documented. 25. There is ponding water in the southeast comer of the west canopy roof at the main entrance (D4). 26. There is still no documentation on the glazing system and installation at the barrel vault. 27. A large percentage of the metal parapet cap needs touch up or repainting. Along both horizontal edges/corners paint is scratched and scraped and the base metal material is visible, as well as along numerous places along the horizontal surface of the parapet. 28. Additional much up painting is required on the metal flashing at the south end of the high wall to the Courtroom (Grid B). It appears an additional coat of green paint is required along the horizontal base flashing. 29. Provide roof membrane or cap sheet protection pads (approximately 3' x 3') at the two south downspouts from the high area of the Court area (Grid B) onto the lower roof level at the point of discharge of the downspouts. 30. Remove the two concrete splash blocks from the canopy roof at the west entry of the 2 building, and provide a 2' x 2' cap sheet protection pads in lieu of. 31. There are significant gaps in the aluminum window frames along the exterior of the perimeter of the Court Room which may provide access for water penetration. 32. Additional caulking/sealant is required at the top metal drip atop the Courtroom window frames around the entire perimeter. There is an exposed gap between the metal drip and the brick lintel. 33. Additional work appears to be needed at the transition sharp comer along the north edge at the intersection of the metal parapet with the Courtroom radiating wall (A-2.8). 34. Additional work appears to be required at the north edge of the radiating beam at the east canopy at the intersection with the vertical brick wall. 35. Additional touch-up paint is required at the radiating beams of the three lower level canopies as there are scratches and flaked paint. 36. Additional caulk and rework is required at the transition of the metal parapet coping at the northeast comer of the barrel vault (C-i). 37. Additional paint is required at the metal trim piece at the north edge of the barrel vault glazing at the base (Grid C). 38. The highest roof level of modified bitumen above the Courtroom could not be accessed nor reviewed, but it will be assumed that the same comments for the other modified bitumen roof installations are applicable for this area. 39. There is a significant gap (up to 1") between the metal parapet coping with the face of the brick veneer around the entire perimeter at the top of the Courtroom roof along the outer edge, due to the metal coping not following the curvature of the wall. 40. Clean all flow-through scuppers of all debris and garbage around the perimeter of the roof. 41. Provide lightening protection at the three exhaust fan hoods at the south roof area, and lighting protection at the four mechanical units and exhaust fan hoods on the north roof area. GROUND LEVEL 1. Neatly trim all weep hole sash cords at all door and window lintels, as well as around the entire perimeter of the building at the base of the brick veneer walls. 2. There are numerous concrete pavement and sidewalk control and expansion joints that need to be completely and thoroughly cleaned of all mud, debris, dirt, etc. and filled continuously flush with joint sealant. 3. Provide sealant at the base of the overhead door steel jambs with the concrete slab at 166C. 4. Provide a floor mounted door stop for Door 166D. 5. There are numerous areas of erosion in the newly seeded grass around the entire site which must be properly infilled and repaired and ground cover established. 6. In approximately a few months the Bermuda grass seed must be planted per the Contract Documents. 7. There is improper drainage from the two downspouts at the southeast corner of the building on the east wall of the SallyPort. The concrete splashblocks at the base of the downspouts are embedded in the dirt and landscaping and do not allow for positive drainage away from the building. 8. Apply continuous sealant at the top of all windows as required to infill and seal all gaps and holes between the metal drips and the brick veneer lintels. 9. Apply continuous sealant at the joints in the concrete pavement with the concrete infills at the pipe bollards outside of Door 166A. 10. Provide a concrete splash at the downspouts in the mechanical yard. 11. Provide grouting to infill the exposed holes around the perimeter of the fire protection system drains along the east wall of the SallyPort behind the emergency generator. Currently there is up to a 4" gap or hole between the escutcheon plates and the brick inffll. Upon infill of solid grout provide continuous sealant at the pipe penetrations through the wall. In addition there is an electrical outlet box at the same location which needs additional grout pointing or sealant and caulking at the exposed gaps. 12. Clean all PVC weeps at the mechanical yard brick wall penetrations, and verify that there are screens installed on the inside face of the PVC weeps to prevent sedimentation and infill blockage within the weeps. 13. In numerous areas there is additional repointing or recaulking required at the base brick veneer joint atop the through-wall flashings. The base mortar joint in numerous areas is chipped and spalled and flaking out and must be repointed. 14. There are numerous areas of erosion at the outside edges of concrete splash blocks at downspouts which must be infilled and repaired and ground cover established. 15. Provide ADA threshold plates at all exterior doors around the perimeter with the concrete stoops (Door 133B, 124B, 105, 100A, l19A, l19B) per Memorandum No. 33, Item No 47). 16. Provide infill sealant at the fire protection penetration at the hinge side of Door 123 through the brick veneer wall. 17. Provide a door stop for Door 123. 18. There is a clean out plate recessed approximately ¥5" below the surface of the concrete stoop near Door 123 in which the edge of the concrete needs to be ground and tooled and smoothed out. 19. Provide continuous watertight sealant around the entire perimeter of all windows and doors at the brick veneer, and particularly along the sill of the windows. Numerous joints need additional and/or touch-up sealant. 20. The gas regulator near Door 123 needs to be painted green to match the rest of the piping. 21. A significant amount of sealant around the perimeter of the exterior storefront units and windows must be redone as it has already sagged and gapped and separated. 22. Additional sealant is needed in order to float out the joint along the storefront outside of Breakroom 105. In addition the rough and irregular concrete ledge at the base of the storefront must be ground and chipped and smoothed out and touched up and epoxy grouted for a smooth condition. 23. Provide additional mortar pointing or sealant at the lintels above all storefront and window units at each end at the extension into the brick joints. 24. Provide continuous sealant around the perimeter of the canopy beam end plates against the brick veneer wall at all three entrance canopy locations. 25. Clean the site of all debris, construction materials, etc. including rebar, insulation, pieces of brick veneer, concrete rubble, etc. 4 26. Provide additional fill against the vertical edge of all concrete stoops and sidewalks. There are numerous areas where there is already erosion and undermining at the edge of the concrete stoop and sidewalks. In addition, all exposed honeycomb and spalled edges of concrete stoops and sidewalks must be epoxy grout patched. 27. There is a spalled and broken piece of concrete sidewalk which must be repaired and replaced at the intersection of the sidewalk from Town Center to the sidewalk at the north end of the Justice Center. 28. There is a sprinkler valve box at the north end just north of the sidewalk which does not sit flush and level with the grade. 29. Properly router and clean out the joints on both side of the red wood expansion joints and infill both edges with continuous sealant at the concrete infills of all the light bollards along the north sidewalk. 30. Repair the bent drip outside of the window on the north edge of the Jury Deliberation Room 101. 31. There is an open projecting piece of PVC pipe approximately 18" above grade along the north wall which does not appear to be utilized for anything. 32. Additional grading and infill of dirt is required at the north end of the site near intersection of the sidewalk with the visitor parking lot. There is approximately a 3" to 4" depression in which water ponds and does not drain properly. 33. Router and chip and clean and tool out the joint between all concrete curbs with the concrete sidewalk/stoop on the redwood expansion joint. Currently the edge is chipped and irregular and spalled and must be epoxy grout patched for a clean and continuous edge. 34. The sprinkler valve box along the northeast comer is recessed/depressed approximately 4" below grade and should be properly adjusted. 35. Provide metal cover plates at all downspout penetrations into the storm water drainage system at areas where there is just dirt fall in landscaping beds. Cover plates were provided at down spout penetrations at concrete stoops and sidewalks, and shall be similarily provided at the penetrations through grade to prevent dirt and debris from falling into the hole and eventually impeding draining through the hole. 36. There are numerous spalled and chipped edges of all concrete paving, concrete sidewalks, and hardscape throughout the site which must be epoxy grout patched. In additional all joints should be completely cleaned of all debris and garbage and continuously caulked. 37. Expansion joint material between the sidewalk along the east and west and the hardscape should be slightly recessed below the surface and continuous sealant provided along this joint. 38. Additional fill and grade work and seeding is required at the strip between the sidewalk along the west side and Town Center Blvd. 39. The water meter cover plate in the sidewalk at the northwest comer needs additional prep work. 40. Irrigation cover plate on the hardscape along the west side at the entry needs additional tooling and touch up at the perimeter. 41. Clean all concrete splatter on light fixture pedestals at the base of the flagpoles. In addition, clean all concrete splatter from aluminum flag poles. 42. Provide additional chipping and grinding and smoothing at the base of the flag poles under 5 the escutcheon cover plates. Provide continuous sealant at the base of the flag poles. 43. Provide continuous sealant around the perimeter of the water hose bib flange at the west side at the entry door 119B. 44. Provide continuous sealant at all steel lintel plates extension onto the brick veneer at all window and storefront units. 45. Provide continuous sealant at the top of all cast stone vertical infill pieces between the storefront units along the west wall at the underside of the steel lintel plates (3 places). 46. Drainage does not appear to be positively sloped away from the face of the building at several landscaping beds throughout the site. 47. Provide a concrete pad/collar around all cleanout plates in sail/grade per Detail 03/P-3. 48. Clean black tar from gutter at southwest comer (J-5). 49. At the projected water piping pad at the southwest comer all edges of the pad (vertical and horizontal) need to be ground and rounded, and were supposed to have been 3/4" by 45 degree chamfers. 50. Remove duct tape from manhole lid frames at southwest comer of site. 51. Clean and remove lettering and markings from electrical switch gear units along the west side of the site. 52. Cut and recess/depress all expansion joint material beneath surface of the sidewalks and street abuttment and fill with joint sealant continuously. 53. At the southwest comer the concrete apron/ramp is spalled and cracked at the transition corner and must be repaired and replaced. 54. Landscaping is missing from the south landscaping bed at the entry door (133B). 55. Them should be a concrete splash block for the downspout at southwest comer of building (J.2-5). 56. Provid9 sealant around the perimeter of the hose bib flange and electrical outlet at the south end of the building near Door 133B. 57. Exposed concrete ledge needs to be cleaned and chipped, and epoxy grout patched at base of storefront unit at Door 133B. 58. There is a large crack in the concrete pavement at the south end of the building approximately 50' out from the Sally Port which may create a future problem. 59. Additional grading and/or dirt needs to be infilled at the concrete flume drainage in the southeast corner of the site and the entire drainage ditch is severely eroded and must be repaired and ground cover established. 60. At the precast manhole lids at two locations on the southwest comer the lifting lug recesses in the top surface should be cleaned out of all dirt and mud, and infilled with an epoxy grout patch to float out flush with the rest of the surface. 61. The transition of the concrete sidewalk with the street at the northwest corner at the entry into the visitor parking - the ramp transition is cracked and spalled and must be replaced. 62. Additional f'me grading and seeding is required at the north end of the site at the drainage channel and swale from the visitor parking lot out to the storm water inlet. 63. All vertical and horizontal edges of concrete pads for switch gear, transformers, emergency generator, etc. throughout the entire site were to be a 3/4" by 45 degree chamfer. The blocks that are underneath the gas line need to be leveled. 2. Disconnects on the units need to be labeled. 3. Gas line has not been labeled. 4. Exhaust fan over the jail has loose squirrel cage and is rattling very badly. 5. Three exhaust fans do not have lightning protection. All other exhaust fans have lightning protection. 6. Rooftop Unit-8: There is a metal bracket belonging to the unit, this should be either reattached or discarded. 7. Public Restroom for Men/Women; Exhaust fan has only been secured with two screws. Contractor needs to add more screws to the curb. 8. Exhaust fan directly to the west of RTU-3 and 4: Cover motor shroud is lacking screws or bolts, the curb attachment has been accomplished with two bolts. This needs to be updated with at least two more screws or bolts to secure the fan on the curb. 9. Ductwork above the entry vestibule needs to be supported. Weight needs to be taken off the ceiling. 10. Outside air intake for RTU-5 needs the bolts completed to the unit. At present there are only two bolts holding down the shroud. Minor puddling exists in the parking ddve on the south side of the building. 2. The flume discharging near the dumpster needs to be backfilled and vegetation needs to be established to eliminate erosion at that area. 3. Minor grading clean-up needs to occur in the area behind the emergency generator. 4. The clean-up in the parking lot in general needs to be completed. 5. Some material needs to be added around the water meter near the water meter vault to bring existing ground up to grade. ~ 6. Some ponding water exists behind the curbs adjacent to the handicapped spaces on the north side of the building. 7. The grates in front of the building need to be unclogged. 8. The decorative paving in front of the building has heaved adjacent to the street sidewalk and portions of the paving show minor cracking. An additional coat of sealer is required at the top and bottom edges of all wood doors. Room 100 - Corridor * Reference General Notes 2, 7, 8. * Install sleeve at fire damper with retaining angles at south wall. 26. Outside edge of threshold at Door 100A has approximately a 3/4" differential with the stoop elevation which is unacceptable. In addition, there is spalled and chipped edges on the outside edges of the threshold much of the epoxy grout patched. (Install ramp threshold.) 27. Touch up paint door frame 100A, the green part of frame and hinge side of door. 30. Recaulk the outside edge of the threshold plate at Door 100A and also provide complete backer rod and caulk at the bottom of the door frame which do not come in contact with the concrete threshold. * Remove loose stud brace above ceiling above hinge side of Door 102. * Ductwork penetrations at three places through one hour wall need retaining or flashing angles on both sides of wall 101, 102, 111. ** Roof leaking near Door 113B. Repair leak and replace stained ceiling tile. ** Door 113B dead bolt not operable. *** Seal wood doors at top and bottom. Room 101 - Conference/Jury 3. Clean caulk and paint overspread around perimeter of both window frames, and provide continuous sealant around the perimeter of the window frames. * Flex duct needs to be sealed. * Install fire damper and retaining angles at west wall penetration. Room 102 - Judge * Reference General Notes 2, * Flex duct needs to be sealed. * Install fire damper and retaining angles at west wall penetration. * Return air not per Addendum No. 1. Room 103 - Court Director * Flex duct needs to be sealed. * Condensate line needs to be supported. * Clean and triha sealant at perimeter of window. Room 105 - Break * Reference General Notes 3, 5, 7 6. Door stops for Door 105 have been fabricated and cut incorrectly at the base. They are approximately undercut by 1" at strike side. 7. There is an approximate 1-1/2" differential from the threshold plate to the exterior concrete stoop at Door 105. This violates ADA/EAB. In addition, the concrete is rough and irregular and spalled at this edge and requires epoxy grout patching and additional floating out of joint sealant. (Install ramp threshold.) 12. Provide continuous sealant around the perimeter of the storefront unit on the north wall, as well as around the window frame on the east side. 24. Clean paint and sealant overspread from window frame on east wall and trim out and provide continuous sealant around perimeter of window frame. 28. There are several vinyl floor tiles which are not properly adhered and loose. 29. There are chips, scuffs and damage to the gypsum board furrdown edge above the cabinets which must be touched up, painted and repaired. 33. Rework and redo sealant joints around three sides of top cabinet since it is cracked and gapped in several areas already. 35. Touch up and redo sealant joint around all edges of the counter splashes (clean overspread). 38. Properly secure front apron at counter and appears to be skewed and is not level and plumb. 40. Apply continuous sealant at the base of the counter panel supports with the vinyl floor on all sides, including in refrigerator and ice maker alcove (or vinyl base). 42. Provide a f'mished out enclosed cover box for the hub drain below the sink per RFP No. 06. (Secure in place and repair delaminated edge.) 49. Front apron beneath sink is not mounted square and plumb and level with respect to distance from wall surface or front counter surface and should be readjusted. Apron installation does not comply with ADA and RFP No. 6. (5-1/2/12) Room 106 - Mens Toilet 2. Provide continuous caulk around door frame. Prior to continuous sealant, properly trim and cut wall fabric to lie flat against wall and abut edge of door frame. 15. Recessed paper towel dispenser/disposal does not sit flush against wall surface. It appears as though ceramic tile cove pieces must be removed to allow this to happen. Room 107 - Womens Toilet 17. Provide continuous sealant around perimeter of door frame. In addition, wall fabric must be properly trimmed around perimeter of door frame to lie completely flat and tight around edge of frame, and then provide continuous sealant. (Hinge side) Room 108 - Court Records * Condensate line needs supports. Roo~n 109 - Court Clerk " 17. Install lock cylinders in the three lap drawers, and all sliding drawings. * There is a significant vibration in the wall and ceiling near the southwest corner and west side. * Provide cushion dots at all four corners of six (6) large file sliding drawers. * Open supply "opening" needs to be covered, ductwork sealed and insulated. * RTU noise and vibration. ** Repair roof leak and replace stained ceiling tile. Room 111 - Corridor * Reference General Notes 2, 7, 8, 11, 12, 13 7. The ceiling along the south end corridor which runs east-west is not level, and appears to be dipped near the south center of the corridor, or possibly the edge track along the south side of the corridor is installed slightly too high. 13. Touch-up paint at top of fire extinguisher cabinet.. 20. * Replace stained ceiling tile near Door 106 and repair leak. * Vibration and rattling above ceiling near hinge side. of Door 106. * Sound attenuation batts above ceiling along west and north walls common to Courtroom 113 have not been properly secured in place. * Install retaining angles at fire damper through north wall at Door 100B. * Repair damaged ceiling grid and tiles at Door 111. Room 112 - Courtroom Corridor * Reference General Notes 8. 5. Adjust the smoke seal at the base of Door 111. 8. Install a smoke seal at the bottom of Door 116, 112B and 112A. (7745 V x 36") 11. Provide appropriate trim-out and flange hole fittings at top of Doors 112B and 112A.. 12. Locking mechanism for Door 112B and i 12A not operating correctly. 19. Additional slate grout is required at joints and at door jamb bottoms at Door 112A. 25. Repair torn and ripped gypsum board at north sprinkler head in the ceiling. * Light fixtures in gypsum board ceiling must be provided with blankets/pillows (three places). * Positively secure loose sound attenuation batts between metal studs above ceiling common to Courtroom on north wall. * Slate is cracked at door receivers at 112A. : * Return air not ducted. Room 113 - Courtroom * Reference General Notes 8, 11 18. Complete installation of all door hardware, smoke seals, etc. at Door 112B. Adjust lock mechanism. 31. Additional work is required at the red carpet accents throughout the room as the carpet tile joints are very visible and noticeable and need additional work. 32. Additional securement and stabilization of the west radiating half height walls is required in the center of the courtroom. 36. Clean paint overspread around the perimeter of the wall hung light fixtures. 46. Provide some sort of infdl and clear sealant at the gaps at all wall intersections within the millwork units, particularly on both sides of the Judge's bench and at the bailiff/jury area. There are up to 1/4" gaps between the intersection of the wall surfaces which must be corrected. (Redo) 52. The bottom edge of the west ramp is incorrect in that the ramp was to have completely sloped down to a 0'-0" elevation at the line coming off the edge of the column furrout and the step to the witness area. Instead there is approximately 1/2" drop along this line. A transition strip will not be possible due to the location of the railing pedestal base plates. (ADA) 54. Provide additional finish out of the vertical tapering surfaces along each side of the ramp on the west ramp. 56. Additional securement is required of the railings at the west ramp. 57. Additional trim-out and termination is required at all edges of the acoustical wall fabric on the center panels in which the fabric is surface mounted to the gypsum board wall surfaces. There are frayed, loose and uneven edges at all vertical and horizontal edges of the reveals which must be properly f'mished out. 58. Provide a trim angle of some sort at the step from the intermediate level to the Judge's level at the exposed corner of the carpeting. (Trim out end of angle) 67. Some sort of brace is required for the countertop at the Court Director's station. 73. Complete installation of both the west and east railing units. (Still doesn't meet ADA) 76. Provide addkional and proper securement of railing at the east ramp to the north wall. 80. There are areas in which the thin veneer sheet on the outer face above the bottom reveal has become delaminated in several areas. 81. Provide protective angle at the edge of the step to the Jury box. 82. Vertical corners at the gypsum board furrout at the building column A. 1 - 2.4 at the ramp in the northeast corner are chipped, damaged, abraded, etc. and must be repaired and touch up painted. 84. Provide some sort of n-im piece at the exposed vertical edge of the gypsum board at the edge of the ramp adjacent to the Judge's bench. (Provide 1/4 round vs. 1 x 2) 85. Provide additional securement of the clear plastic corner guards at the wing wall in the northeast corner. 86. Complete installation of the data/communication speaker coverplates beneath the bailiff station. 90. Some sort of transition strip will have to be provided at the end of the ramp on the northeast corner as there is an approximate 1/2"-3/4" drop. Ramp was to have completely terminated back down to 0'-0" at this location. * Sound attenuation batts on the east and south ends of Courtroom not properly secured in place. * Replace broken light fixture cover. * Additional trim out and closure required at wood trim and step angle at west ramp. * Finish out exposed edges of end counter supports visible at Bailiff's station. Install missing front apron piece similar to Witness counter. * Buzzing light in northeast corner. Room 114 - Counselor 1. Door is chipped and splintered at base and there are several scratches and gouges on the door surface. * Positively secure loose sound attenuation batts between metal studs above ceiling common to Courtroom on north wall above ceiling. ** Door smoke seal drags. Room 115 - Counselor * Positively secure loose sound attenuation batts between metal studs above ceiling common to Courtroom on north wall above ceiling. 4 Room 116 - Janitors Closet 12. There are several vinyl floor tiles loose in the south end of the janitors closet. * PoSitively secure loose sound attenuation batts between metal studs above ceiling common to Courtroom on north wall above ceiling. * Touch-up paint ceiling. Roo~n 117 - Women's Restroom 4. All toilet partitions and specialties were to have been installed with vandal-proof fasteners as this is a "public" restroom. Only about half of the fasteners are vandal-proof, and the rest are regular standard Phillips head. 7. Properly clean under all stainless steel shoes at bottom of toilet partitions pilasters. 12. Door into toilet area is warped and does not sit and close tightly and flush within door frame. (Replace Door 117) 14. The door frame for the door was incorrectly prepped for a lock/latch and trim plate must have a rolled edge and be painted. 23. Provide proper trim out and termination of the exhaust and return air grilles along the vertical face of the light trough. 39. Additional rework is required around the perimeter of all edges of the splashes with the ceramic tile. * Lights at trough on west and south wall need to be supported properly. Room 118 - Men's Restroom 5. Additional stabilizer bars are required for toilet partitions, as it is not properly secured and racks severely. 8. Additional touch up and work is required on the gypsum board ceiling at the edge of the recessed lighting trough on the east wall. In several areas, the j-bead is visible and/or cracked and needs additional taping and bedding and paint. 11. The return/exhaust air grilles in the vertical face of the light trough need to be properly finished out and terminated. 12. All the fasteners for the toilet partitions and specialties were to have been vandal-proof fasteners as this is a "public" restroom. 17. Additional work is required behind the building column along the west wall. There is visible debris and taping and bedding material between the column and the face of the ceramic tile at the base. Additional joint sealant is required around the entire perimeter of the column at the ceiling penetration. In addition. there are cracks in the gypsum board along the edge of the column penetration at the ceiling which must be repaired. A ceramic tile cove base piece must be fitted in the base of the wall behind the column. Caulk atop ceramic tile wainscot behind column. 24. Door frame/door is warped and does not fit flush within frame upon closing. Replace Door 118. 26. Door frame improperly prepped for latch/lock and must be corrected. (Filler plate should have rolled edge.) (touch-up paint trim piece) 37. Additional float out and trim out is required at joint sealant around perimeter of counter splash and front apron. 40. Counter is mounted too high at east end and lip of sink exceeds 34". * Lights at troughs on east and south wall not properly supported. Room 119 - Vestibule * Reference General Notes 3, 5, 7, 10 1. Additional touch up work is required around the sprinkler head escutcheon plates on both the north and south walls. (secure) 2. Verify that the fire alarm system annunciator panel is operable and working. 7. Clean the vertical joints along both sides of both building columns of all taping and bedding material and sealant, and touch up paint is required, and-apply continuous backer rod and sealant full height on both sides of the building columns. (Clean and touch-up paint.) 8. The three recessed ceiling lights need additional adjustment and rework since the collars do not set up tight and flush against gypsum board ceiling surface. (Clean paint overspread) (Center fixture not working) 10. Verify correct operation and installation of all door hardware, door monitor systems, etc. for the four sets of double doors at the Vestibule specifically panic bar rod mechanisms and locks. 11. There is an exposed and abandoned screw holes (3) in the storefront frame in the center section at the west storefront system at top. 12. Touch up and trim out continuous sealant around entire perimeter of both storefront units. 13. At the south building column on the back side there is a piece of metal plate which should be removed and slate continued in toward the throat of the column. 15. Touch up all scratches and abrasions on the storefront unit. 18. Provide additional grinding and smoothing of the exposed concrete edge outside of Doors 119A and l19B. 19. Touch up and level and float completely flush the sealant joint along the west edge outside of the doors. 22. Provide a dust proof receiver at the threshold for the door lock mechanisms for Door 119B and 119A, 12lA, and 12lB. 24. There is more than a 1/4" differential from the threshold plate to the concrete walk outside Door 119A and 119B which violates ADA. (Install ramp threshold.) 25. Touch up and adjust properly all gasketing for the storefront units. 26. Clean the storefront units of all paint over spray, sealant overspread, etc. 27. One of the door leafs at Door 12lB is scratching the slate floor. The slate floor shall be repaired or replaced and the door properly adjusted. 30. Additional repointing and much up of mortar joints in the slate is required along all threshold edges of all doors. 32. There are abandoned screw holes in the heads of Doors 12lA, 12lB, l19A and l19B. 33. Door gasketing weatherstripping needs to be properly adjusted as there is visible daylight between the door leafs in several areas. 35. Apply additional repointing and infill of grout joint at slate floor at base of east storefront unit between doors. 38. Clean area above ceiling of Vestibule of all dirt, debris, excess construction materials, scraps, etc. 40. Above vestibule ceiling, completely clean all dirt and debris and garbage. In addition, correctly position all batt insulation atop vestibule ceiling. Currently, batt insulation is torn and ripped and discontinuous in several areas, particularly at the duct diffuser penetrations. In addition, the flex duct insulation is torn and ripped and caved at several areas and must be repaired. 41. Properly infill and repair holes in gypsum board wall along the west wall above the ceiling. 42. Entire ceiling area above shall have continuous uninterrupted layer of batt insulation with foil facing throughout the entire ceiling area excluding atop the light fixtures and atop the return air grille. 43. File and cut offprotmding sharp screw points in gypsum board track above ceiling directly above Doors 12lA and 12lB, as they will be potentially dangerous for future maintenance personnel. * There were to have been no receiver/latch holes in threshold plates of Doors 119A and 119B. ** Door 12lB is loose. 6 ** Verify magnetic lock operation on Door 12lA. Room 121 - Lobby * Reference General Notes 3, 5, 7, 11. 2. Provide additional infdl and repair and repointing of slate mortar joint at threshold plates of Doors 12lA and 12lB. 4. Properly adjust and positiop~all gasketing of all storefront units and window units throughout the entire Lobby and Vestibule. 19. There are several joints throughout on the slate floor which require additional retooling or repointing. 29. Properly adjust and secure the sprinkler head escutcheon rings in the gypsum board throughout on the north walls. 37. There are two abandoned holes in the of leaf of Door 133A which must be repaired at the relocated door closer. 38. Provide proper recessed rim devices at the holes in the top of Doors 133A. 57. Provide sealant at the perimeter of the counter boxes sitting atop the lower counter edge along the north wall. 63. Provide continuous sealant at the base of the window frame atop the plastic laminate at Dispatch, as well as the vertical joints of window frame at the building columns for a flush and floated out condition. 65. There are significant gaps up to 3/4" on the slate flooring which does not extend properly to or beyond the base cabinet at the millwork unit at Dispatch. (Clean millwork bottom reveal) 66. Provide continuous sealant around the perimeter of all frame sides at Dispatch. 69. The underside of the millwork unit to Records 132 is not covered with plastic laminate similar to the other four installations. 72. The recessed light trough and top surface above the gypsum board furrout above Doors 112A and 133A must be completely cleaned of all taping and bedding material, dust and dirt, and trash. 74. Provide continuous sealant at north and west between top of furr-out and barrell vault window frames. * East side at top of furr-down/out 6" gap between furr-out and bottom chord of truss. Infill and paint. * Clean and caulk frame at storefront along north and west walls at barrel vault. * Paint white gaps at truss top chord between window frames on north side. *** Thermostat not working properly. Room 122 - Storage * Reference General Notes 2, i 1, 12 4. There is black paint overspray and droppings in several areas that must be touch up painted (at top of door frame). * Opening in return air ductwork (close, seal, and insulate). * RTU noise and vibration. Room 123 - Electrical 5. All penetrations through all gypsum board surfaces should be completely sealed and caulked around. (Touch-up and redo.) 8. Fire protection line in northeast corner should be properly braced and secured to wall. 15. There is approximately 1" drop in the concrete floor at the threshold of the door which violates ADA. 17. Upon installation of threshold plate for door completely caulk and seal both sides of the threshold plate, and the outside edge requires epoxy grout infill and leveling beneath the base plate. 24. Provide a floor mounted door stop for the door to prevent door from swinging in and hitting edge of brick veneer. Room 124 - Multi-Purpose/Training * Reference General Notes 3, 5, 8, 11. 2. Provide sprinkler head escutcheon ring at head in the northeast corner of the room. (Secure flush to wall.) '~ 13. Provide a floor mounted door stop for the inactive leaf at Door 122. 18. Additional touch up work is required at the sprinkler head plates at the gypsum board furrdown ceiling, as there are visible rough edges and gaps. In addition, the sprinkler head plates need touch up and cleaning. 23. Install all blind systems throughout the room per the construction documents (horizontal, vertical, and motorized). 24. Additional touch up and rework is required at sealant at perimeter of storefront units on both the east and west walls. 26. Some sort of finish out will be required at the vinyl base intersection with the west storefront unit on both sides. Return the base to the face of the frames. (Pieces installed are not acceptable) 32. There is an approximate 1-1/4" drop off at the threshold plate of Door 124B which is unacceptable and does not comply with ADA. A maximum of 1/4" differential is allowed by ADA. Install ramp threshold. 33. Touch up all scratches, blemishes, etc. on all storefront windows and wall systems throughout the entire room, including the barrel, vault system. 34. Additional touch up and floating out for a smooth transition is required between the gypsum board wall surfaces at the reveals of both the north and south walls, as these transitions are very rough and irregular and unfinished at ends/corners/ 40. Additional f'mish out and cleaning and trim out is required at the fire extinguisher cabinet on the south wall on both sides with the reveal. This should either be floated out flush and solid on each side and painted, or all debris and taping and bedding material removed to clean the joint. 55. The communication/dam outlet and the blank-out cover for the cable outlet locations on the north wall have been interposed. 58. * Provide continuous sealant at furrdown ledge with window frames on north and east sides. Clean window frames and fill gap in storefront frames. * Paint J-Box and conduit at top in northwest corner. * Touch-up paint and clean sealant overspread at vertical joints full height on each side of columns C-1 and D-1. *** Provide all required accessories for marker boards (map rail, map hooks, display rail). *** Retractable door at ceiling projection screen not working properly. Room 125 ~ Dispatch * Reference General Notes 8, 11. 31. Several of the vinyl floor tiles in the northwest corner are not properly adhered to the concrete floor. 34. Additional touch up paint required at building column D22 northwest corner. 35. Apply continuous sealant around the perimeter of both window units in the northwest corner at building column D-2. 37. There is approximately a 1" gap in the gypsum board at the underside of the counter in the northwest corner which should be properly infilled. (Exposed wood trim not acEeptable) 40. Provide backer rod and the sealant at vertical gaps with window frames at building column D-2. 42. The storefront framing pieces at the base of one of the window units at the northwest corner is gapped significantly, as well as along the top. 45. From edge of the counter at the window unit does not extend down sufficiently to cover and conceal the counter braces/backing. Provide some sort of additional trim out to conceal or finish out the visible counter supports (similar to Courtroom counters). 49. The bottom of window frame at the paper pass was to have been provided with a bottom closer panel. As currently installed, paper or material may catch up in the recessed throat of the frame at the underside. * Remove scraps and debris and wood planking above ceiling in northwest corner above Dispatch window. ' * There is a fire protection sleeve penetration through the south CMU wall in southeast corner which must be fire caulked at annular space. *** Verify and program annunciator panel and shunt switches for door monitor system for Doors 10lA, 105, 111, 133A, 12lA, 123, 124B, 127, 133B, 162A, 162B, 166D, 167, 100B. Provide gaining for door monitor system. *** Lock/latching light at annunciator panel in control console not working properly for Security Vestibules 178 and 185. Room 125A - Telephone Room 7. Provide plates for conduits not yet utilized, and at all other conduks upon f'mal installation provide a sealant/foam infill at top of conduits to make weathertight and airtight. Room 126 - Equipment * Provide retaining angles at duct penetration through roof deck above ceiling. Foam inf'fll penetration through roof and roof deck, and make weathertight and weatherproof. * Replace/repair joist bracing angle cut for exhaust duct installation above ceiling. Verify that fh:e shutter assembly is operable and interfaced correctly with the fire alarm system. 20. At the east side of the south counter there is an approximate 1/4" gap between the counter top and the surface of the gypsum board which must be properly infilled. 21. The top flange piece above the hood appears to need adjustment and proper securement to lie flush with the gypsum board wall surface. 23. Provide some sort of rubber bumpers at the base of the coiling door so it does not scratch and damage the counter surface. 25. There is a crack in the counter top plastic laminate off the corner of the gypsum board at the opening at the south end of the coiling door. 29. The radiating wall segments are also to sit flush down to the gypsum board wall surface and not be gapped up to 1/2" and filled with sealant. 15. Additionally finish out is required and black sealant should be provided at every one of the recesses into the shelving units for the rails. Provide continuous black sealant along the top edge of the metal nametag rails at the exposed edges of the shelf ledges. 25. There are numerous places where there are chips and delamination of the corner edges of the mail shelving unit which must be repaired. 26. A large percentage of the mail cubicles result in gaps up to 1/8" between the vertical and horizontal 13 pieces. They need to be sealed and finished off at all of these individual joints. 29. HOLDING CELLS, SECURITY VESTIBULES, INTOX, KITCHEN, VISITATION, AND BOOKING 1. Provide firecaulking at all penetrations (open conduit, abandoned conduit, roof joist chords, pipes. sleeves, etc.) through the north, west, and south CMU walls as this is a two hour rated partition at the perimeter of the Jail area. 2. Clean all debris, garbage, mortar/grout pieces, scrap materials, dirt and excess construction materials from atop the ceiling area (concrete and gypsum board). 19 3. Provide additional trim-out and touch-up firecaulking at top of CMU walls at underside of metal roof deck at the entire Jail perimeter walls. 4. Properly secure lights/ducts/grilles/etc., penetrating the ceiling throughout the Jail area, and independently support to roof structure. Additional support wires required. 5. The security cameras mounted in the gypsum board ceiling are only secured with three (3) toggle bolts. This is unacceptable! Mount and secure cameras per 11198-2.7.C criteria. 6. Duct insulation torn and ripped in several areas and must be repaired. 7. Cut off flush all vertical protruding rebar from all CMU walls for safety purposes (future maintenance personnel). 8. Additional pipe saddles required at pipe supports and insulation. 9. Additional pipe supports required. 10. Flex duct is pinched and caved and not properly supported and is sagging and draped over top of pipes and conduit. 11. Flex duct is not secured in place atop flange boxes at grilles and ceiling penetrations. 12. Fire protection lines need additional supports at drops/heads. 13. Fire protection needs escutcheons at ceiling penetrations. 14. Valve insulation needs to be completed. 15. Valve tagging needed. 16. Sheet metal boxes at grilles are not externally insulated. 17. Access doors are needed to all valves and dampers per Specification 15010. 18. Additional horizontal leg length required at several flanges at duct boxes into cells in order to properly secure/anchor to concrete ceiling. Secure all duct boxes in place to flanges, and flanges to concrete deck. 19. Firecaulk vertically on both sides of building column F-3.1. 20. Inffll with firesafmg and fireputty gaps beneath and atop beam on south wall along grid G to provide a complete rated envelope. GENERAL NOTES (Applicable for all rooms) 1. Adjust/rework vinyl base throughout all rooms at all joints. At vinyl base joints, sections are visibly offset and irregular and do not result in a continuous appearance. 2. Clean all paint overspray, overspread, dirt, grime, and foreign material from suspended ceiling grid/track. 3. Remove all orange dots and other labels at base of storefront unit doors at the inside edges and clean. 4. Remove all temporary masking tape door designations throughout the entire building. 5. Properly adjust all gasketing along all storefront units throughout the building. There are numerous locations, particularly in the corners in which the gasketing has been pinched and forced inward and results in uneven appearance. Properly adjust all the gasketing for a consistent edge around perimeter of all glazing units. In addition, clean and remove any taping and bedding, sealant, dirt, etc. from along the edges of the gasketing at the perimeter of the windows. 6. Clean all scuffs, smudges, streaks, paint overspray/drips, taping and bedding material, and dirt off all vinyl bases throughout the entire building. 7. Properly adjust all door hardware throughout the entire building, including proper adjustment of door closer mechanisms, door strike/latch/lock interface, etc. Numerous key entry in lock cylinders are damaged and prevents entry of key. In other instances, the lock cylinders accept the key but do not allow rotation of the key. A hardware representative shall inspect all door hardware configurations and installation and repair deficiencies accordingly. All locks shall be keyed as directed by the City of Coppell in Memorandum No. 14. Storefront doors/locks currently are not keyed on master/grand master system. PSA requests certification from hardware supplier that all installations are correct and operable. 20 8. Top and bottom edge of all wood doors shall be coated/finished similar to rest of wood door surfaces, i.e. prepped and varnished. 9. Vinyl floor base at interior gypsum board wall corners does not fit tightly and flush back into the corner radius resulting in an approximate 1/4" gap between the comer of the walls and the vinyl base curvature. 10. Provide additional trim out and touch up at perimeter of all door frames (in gypsum board and CMU walls) throughout the building for caulking/paint overspread resulting in irregular and wavy lines at the perimeter of doors. Provide continuous sealant around entire perimeter of doors, including the top edges, and paint to match. 11. All hinge pins for all doors must be completely tapped down and properly secured and seated in place. 12. Acoustical ceiling tile paint overspray, overspread, taping/bedding material, and splotches must be completely cleaned off all ceiling tiles, particularly at the perimeter of all rooms. If paint cannot be completely and satisfactorily cleaned off ceiling tiles, the tiles must be replaced. 13. For all wood doors, remove all foreign material, streaks, caulking/paint, etc. Touch up varnish as required. Repair all gouges, scratches, gashes, etc. on wood door surfaces or replace doors damaged beyond repair. 14. Clean paint overspray/overspread, sealant overspread, taping/bedding materials, paint drops, pencil/marker lines/dots, streaks, dirt, grime, etc. from all light switch coverplates, electrical/communications/data telephone coverplates, window and door frames, doors, hardware, specialties, millwork, wall and ceiling surfaces, etc. 15. Sealant/caulking throughout project is very poor and inconsistent. A large percentage of existing sealant is separated, cracked, sagged, gapped and must be removed and replaced. At other areas the sealant is not continuous along edges. Sealant overspread has not been cleaned or trimmed. There is still a large percentage of sealant to be installed. Sealant shall be provided at all joints and intersections of all dissimilar materials. 16. Texturing of gypsum board wall and ceiling surfaces is very inconsistent and irregular throughout the rooms, particularly at gypsum board vertical and horizontal corners, furr-outs/downs, outlets/switch coverplates, etc. 17. Provide cushioning dots/pads for all millwork drawers and doors. 18. All hollow metal doors require additional work. Thoroughly remove, trim-out, and clean aH taping/bedding material, paint and sealant overspread from perimeter of door frame return edges. Touch-up paint door frame returns including top edge and hinge side returns near wall corners, and at bottom of frames at vinyl base. Provide continuous sealant around perimeter of door frame return. including top edge. Sealant/paint lines at intersection of wall surface with door frame return should be a true and straight crisp line, and not a wavy and irregular edge. 19. Install lock cylinders in all sliding drawers of millwork units. Also provide black vinyl cable ports in millwork counters with open knee space beneath. 20. Touch up all scratches, abrasions, etc., in aluminum storefront units. 21.
