I would like to apologize that this was not received sooner. It
must have been lost in the mail since it was sent on July 1, 1986.
The system carries a one year warranty on parts and labor, how-
ever preventative maintenance is not covered under warranty. A
preventative maintenance proposal is attached.

cover the air conditioning system installed at 255 W. Pkwy Blvd.

Your heating, ventilating and air conditioning system
requires preventative maintenance as follows:

1. Change filters
2. Inspect for refrigerant leaks
3. Lubricate necessary parts
4. Check safety devices
5. Adjust all belts
6. Check all temperature controls
7. Check compressor oil
8. Check voltage and amps
9. Check coil surface (evaporator and condenser)

Answer calls for services as promptly as possible

Instruct you in the operation of equipment for
greatest operating efficiency.
$264.00 per maintenance call: 6 times per year.
For other service work required by your system, or for
emergency attention, please call us at (214) 350-2331,
or nights and weekends (214) 521-4111.
It is understood that this agreement is cancelable by
either party by giving thirty (30) days written notice.
