City of Coppell Post Office Box 478 Coppell, Texas 75019 Re: Village Parkway Pump Station Addition Final Inspection Dear Mr. Griffin: A final inspection of the Village Parkway Pump Station Addition was conducted on Friday, May 23, 1997. In attendance were: Mr. Matt Cast, City of Coppell Mr. Larry Davis, City of Coppell Mr. Mike Daigre, North Texas Municipal Construction Co. Mr. Bill Bigham, Noah Texas Municipal Construction Co. Mr. A. Boone, Oslin Nation (pump supplier) Mr. Gary Hendricks, P.E., Shimek, Jacobs & Finklea Following items were found to be in need of completion or correction: 3. Motor louvers on west wall do not fully open when ventilation system is activated 4. Ventilation system requires balancing to prevent air circulating back into the existing pump station vents 5. A screen or guard is required on the ventilation fan (EF-4). 6. Pump No. 5 is still louder than pump no. 6. We have not yet received the noise test on this pump nor the report on the balancing of this pump from the manufacturer 7. Pump No. 5 -- power conduit casing is ripped, burned or melted and should be replaced or repaired. 8. Pump No. 6 was rebuilt recently. We have not yet received the written report indicating the problem and corrective measures taken by the pump manufacturer's representative. 9. General site clean-up and debris removal is required 10. Re-seal the expansion joint at the new driveway connection 11. Route and seal the cold joint at the noah end of the disinfection system concrete slab 12. Some of the bolts holding down the disinfection system housing are loose. 13. The eyewash basin bowl is cracked and needs replaced. 14. The disinfection system valve control panel needs to be braced horizontally to the brick screening wall 15. Trim the trace wire at the valve control panel piping Mr. Kenneth M. The NH3 gas tanks must be secured to the scales utilizing a chain 18. Label the gas mask on the C12 and NH3 emergency gas mask stations 19. Fine grade and re-seed areas disturbed by construction, paaicularly the area behind (west of) the disinfection screening wall, between the existing ground storage tank and access drive, and along the trench line for the DWU sample and solution lines. 20.There appears to be a leak at the DWU sample and solution injection vault. 21.Seal the SCADA System antenna conduit penetration into the Pump Station building 22. The trench for the under ground electrical conduit which was re-routed to the east side of the building in this contract, has settled near the existing electrical manhole. 23.General clean up of the walls and floors in the new pump station room is required. 24. Motor Control Center LC -- the existing heater unit was relocated to accommodate this new motor control center. The old suppoaing bolts and hardware need to be cut away and the tile patched. 25.Add the required flush valve to the strainer in the cone valve control cabinet (See Matt Cast or Ted Dutt) 26. Refer to Noah Texas Municipal Construction Co., shop drawing submittal No. 5050-1 -- Miscellaneous Steel, dated 6/04/96 (copy attached). The City has requested that the opening across the center of the MCC platform in the new pump station have rails instead of the chain and that a handrail be added to the noah wall to assist in climbing the ladder relocated there by change order. NTMCC indicated they do not have adequate railing to accomplish this request. Drawings E1 and E2 in this submittal shows a 90" long aluminum handrail to be connected to the noah pump station wall with three connectors. It also 42 aluminum post and three 60" long handrails along the proposed stairway. None of this railing was constructed when the stairs were eliminated and the ladder relocated. It seems from these aluminum pieces and fittings not used that adequate handrail material is available to construct the 3-24" bars across the old ladder opening and the handrail at the new ladder location. Also, this submittal shows a safety chain across the ladder opening at the south side of the elevated platform. We did not notice this chain during our final inspection. 2q On Tuesday, May 27, 1997, Matt Cast called to inform me that again pump no. 6 has failed out due to high motor winding temperatures. Could the RTD be bad ?? Please check this out and repoa. Once this punch list is completed to the City's satisfaction, we will issue a letter of completion and request the contractor complete the affidavit of final payment and waiver of lien.
